摘要
目的研究体外受精促超排卵过程中激素水平的变化,探讨激素水平与临床妊娠的相关性。方法476个IVF-ET及ICSI周期,应用单因素和多因素Logistic回归分析,研究患者的基础FSH、LH、E2、P,启动日LH、E2、P,HCG注射日LH、E2、P与临床妊娠的相关性。结果:单因素Logistic回归分析:基础激素水平、启动日激素水平和HCG日E2与临床妊娠未见明显相关性,HCG日LH、P与临床妊娠明显相关。多因素Logistic回归分析:HCG日LH、P与临床妊娠明显相关(P分别=0.020、0.020,Exp(B)分别为1.147、0.766),回归方程为Logit(P)=-0.502+0.138*(HCG日LH)-0.266*(HCG日P)。结论HCG注射日LH、P与临床妊娠具有明显相关性,LH水平过低或P水平过高对临床妊娠不利。建议当HCG日P水平过高时,可以考虑取消新鲜胚胎移植,择期行冷冻胚胎移植。
Objective: To investigate the correlation between hormone level and clinical pregnancy rate in in vitro fertilization. Methods : 476 IVF - ET and ICSI cycles were analyzed retrospectively. Applied single - variate and multiple - variate logistic regres- sion analysis to analyze the correlation between basal hormone ( FSH, LH, E2, P), hormone on the starting day ( LH, E2, P), hormone on the day of HCG administration ( LH, E2, P) and clinical pregnancy rate. Results: Single - variate logistic regression a- nalysis: there wasn't significant correlation between basal hormone, hormone on the starting day, E2 on the day of HCG administration and clinical pregnancy rate. But LH, P on the day of HCG administration had significant correlation with clinical pregnancy rate. Mul- tiple - variate logistic regression analysis : there was significant correlation between LH, P on the day of HCG administration and clini- cal pregnancy rate ( P = 0. 020, 0. 020, respectively; Exp (B) = 1. 147, 0. 766, respectively) . The regression equation is: Logit (P) = -0. 502 +0. 138* (HCG 日 LH) - 0. 266* (HCG 日 P) . Conclusion: There is significant correlation between LH, P on the day of HCG administration and clinical pregnancy rate. Excessive low LH or excessive high P on the day of HCG administration is disadvantage to clinical pregnancy rate,
