Effect of Exercise on Diabetic Peripheral Neuropath

摘要 目的:研究长期的运动对I和II型糖尿病周围神经病变的影响。方法:83名糖尿病参与本研究。随机分为两组:即运动组43名,完成4h/W的轻快的跑台上行走,运动强度以50%-85%的心率。对照组40名病人。测量患者下肢振动知觉阈(VPT),神经传导速度(NCV)和神经动作电位振幅(NAPA)。结果:发现两组腓侧和腓肠肌运动神经元的NCV有显著差异(P<0.001)。结果:对照组与运动组比较,发展为运动神经病和感觉神经病的百分比显著增高(分别为21%和0.0%,P<0.05;32%和4.3%,P<0.05)。对照组与运动组比较,VPT增加明显(32.1%和13.6%,P<0.05)。结论:长期的有氧运动可以预防DPN的发生和发展。 Background: Diabetes is the most important cause of peripheral neuropathy (DPN). No effective treating methods have been found. Objective: Examine the effect of long - term exercise training on the development of DPN in both types of diabetic patients. Methods: Eight - three diabetic patients without signs and symptoms of peripheral DPN were enrolled, randomized, and subdivided into two groups: 43 diabetic participants who performed a prescribed and supervised 4 h/week walking on a treadmill at 50% to 85% of the heart rate reserve (EXE) ,and a control group of 47 diabetic participants (CON). Vibration perception threshold (VPT), nerve conduction velocity (NCV), and nerve action potential amplitude (NAPA) in the lower limbs were measured. Results: We found significant differences on △ (delta) in NCV for both peroneal and sural motor nerve between the EXE and CON groups during the study period( P 〈 0.001, for both). Research findings : The percentage of diabetic patients that developed motor neuropathy and sensory neuropathy was significantly higher in the CON than in the EXE group ( 21% vs. 0.0% , P 〈 0.05, and 32% vs. 4.3 % , P 〈0.05 ,respectively). In addition, the percentage of diabetic patients who developed increased VPT (25V) during the study was significantly higher in the CON than in the EXE group (32. 1%vs. 13.6% ,P 〈0.05). Change on Hallux VPT from baseline to the end of the study was significantly different between the EXE and CON groups ( P 〈 0.05) ; no significant change in Malleolus VPT between the two groups occurred. Conclusions: This study suggests, for the first time, that long - term aerobic exercise training can prevent the onset and development of DPN.
