周围型肺癌螺旋CT征象与VEGF表达的相关性研究 被引量:6

摘要 目的探讨周围型肺癌螺旋CT征象与血管内皮生长因子(VEGF)表达间的关系。方法对58例经手术或穿刺病理证实的周围型肺癌患者行螺旋CT扫描,手术或穿刺标本采用免疫组织化学SP法检测肿瘤组织中VEGF蛋白表达,将螺旋CT征象与VEGF表达进行对照。结果螺旋CT判断胸膜侵犯准确率为86.67%,淋巴结转移准确率为88.24%,TNM分期的准确率为81.25%。VEGF的阳性表达率为63.34%,与病理分级显著相关(P<0.05),而与病理类型无关(P>0.05)。VEGF的表达与周围型肺癌的毛刺征、棘突征、血管集束征、空洞征、胸膜侵犯和淋巴转移明显相关(P<0.05),还与周围型肺癌的强化程度密切相关(P=0.035,rs=0.533)。结论螺旋CT可清晰显示周围型肺癌的各种征象,较准确地反映肿瘤浸润转移的病理学和生物学特性,从而指导临床选择适当的治疗方案和对预后的判断。 Objective To investigate the correlation between spiral CT features and expression of VEGF in peripheral pulmonary carcinoma (PPC). Methods Spiral CT scans were performed in 58 patients with PPC which were confirmed by pathology. Expression of VEGF in 58 cases was examined with SP immunohistochemical techniques and compared with Spiral CT findings.Results On spiral CT scan, the accuracies in diagnosing invasion of pleural and detecting for lymph node metastasis were 86.67% and 88.24% respectively,whereas the accuracy of staging in TNM was 81.25%. The abnomal expression rate of VEGF was 63.34%.The abnormal expression of VEGF was associated with the CT features of PPC, including lymph node metastasis, pleural invasion, cavity sign, spicular sign, vessel convergence sign and enhancement degree. Conclusion Spiral CT can clearly show signs of PPC and accurately reveal the pathological and biological characteristics of invasion and metastasis in PPC, and guide clinical therapy and evaluate the prognosis.
