摘要
目的:观察不同时间接受康复治疗对股骨颈骨折患者术后动态骨量及愈合时间的影响。方法:选择近期进行加压螺钉联合带血管蒂髂骨瓣转移手术治疗股骨颈骨折患者65例,入选对象分别在术后不同时间接受了康复干预治疗,同时进行跟骨定量超声(QUS)检查和骨折愈合时间评估。结果:两组患者手术前BUA、SOS和STI等指标无明显区别。与术前结果比较,早期干预组术后1和6个月时BUA、SOS和STI等指标略有下降,但无明显区别(P均>0.05),而对照组1和6个月BUA、SOS和STI等指标均明显低于手术前,同时上述各指标也明显低于早期干预组同期检查结果(P均<0.01~0.05)。早期干预组平均骨折愈合时间为(16.82±5.71)周,明显短于对照组(18.50±7.26)周(t=2.075;P<0.05)。结论:尽早开始康复干预治疗可明显减少股骨颈骨折术后患者骨量丢失及缩短骨折愈合时间。
Objective: To investigate the rehabilitative treatment on dynamic bone mass and healing time of patients with transcervical fracture at different time points. Method: 65 patients with transcervical fracture who received compression screw combining with iliac bone angio-stalk transfer were selected in this study. Rehabilitative treatment, calcaneal bone examination by quantitative ultrasound system (QUS), and healing time evaluation were performed at different postoperative time points.Results: There were no significant differences in broadband ultrasound attenuation (BUA), speed of sound (SOS), and Stiffness (STI) between two groups before surgery. Furthermore, BUA, SOS, and STI in the intervention group were decreased 1 and 6 months after surgery, but there were no significant differences as compared with those before surgery (P〉0.05); while, BUA, SOS, and STI in the control group were significantly lower 1 and 6 months after surgery than those before surgery; additionally, those indexes in the control group were significandy lower than those in the intervention group (P〈 0.01-0.05). Average healing time of fracture was (16.82±5.71) weeks in the intervention group, which was significantly shorter than that in the control group [(18.50±5.71) weeks, t=2.075, P〈0.05].Conclusions: Rehabilitative treatment can apparently relieve bone mass loss and shorten healing time of fracture in transcervical fracture patients as early as possible.
