Expression and significance of p16^(Ink4a) and cyclin D1 in large intestine carcinoma

摘要 目的探讨大肠癌p16Ink4a和周期素(cyclin)D1的表达及其临床病理指标和预后的关系。方法应用免疫组织化学SP法检测大肠癌及正常大肠黏膜中p16Ink4a和周期素D1的表达水平。结果大肠癌组织中p16Ink4a和周期素D1表达率为57.35%和73.53%,而对照组为85.00%和0.00%,两组表达阳性率差异有统计学意义(P<0.01)。p16Ink4a和周期素D1表达与大肠癌分化度、淋巴结转移、临床分期有关。大肠癌中p16Ink4a与周期素D1蛋白表达呈负相关(P<0.01)。结论大肠癌发生机制涉及p16Ink4a和周期素D1调节通路中多个基因的异常,且与大肠癌分级、淋巴结转移及临床分期等有关。周期素D1蛋白高表达可能是大肠癌发生中的早期分子事件,可作为大肠癌早期诊断的标志物。p16Ink4a低表达反映了大肠癌恶性进展,是有价值的预后因子。 Objective To evaluate the relationship between the expression of p16^Ink4a and cyclin D1 and the clinicopathological parameters in the patients with large intestine carcinoma. Methods The SP immunohistoehernical method was used to detect the expression of p16^Ink4a and eyclin D1 in large intestine carcinoma and normal large intestine mucosa. Results The positive expression rates of p16^Ink4a and cyelin D1 protein in large intestine carcinoma were 57. 35% and 73.53%, which were 85.00% and 0.00%, respectively, in the large intestine normal tissues. Difference was significant between those in large intestine normal tissues and those in the large intestine cancer (P〈0.01). The expressions of p16^Ink4a and cyclin D1 were significantly relatied with differentiated grades, lymph node metastasis and clinical stages of the cancer. The expression of p16^Ink4a was negatively correlated to that of cyclin D1 (P〈0.01). Conclusion Abnormality of many genes involved in the regulatory pathway of p16^Ink4a and cyclin D1 may be involved in the molecular mechanism of large intestine carcinogenesis, which is related to the histological grade, lymph node metastasis and clinical stages of the cancer. The overexpression of cyclin D1 may be an early molecular event in large intestine carcinogenesis and may be helpful markers for early diagnosis of large intestine carcinoma. The reduction of p16^Ink4a protein may be an useful indicator for reflecting the progression and high grade malignancy and predicting prognosis of large intestine cancer.
