摘要
为了提高肾损伤的诊治效果，总结１８０例肾外伤诊治体会。闭合伤１６８例，开放伤１２例；合并伤７０例（腹部合并伤４８例占２６．７％）。Ｂ超检查阳性率８７．８％，对重度肾损伤及腹腔脏器伤的检出率１００％。非手术治疗７９例（４３．９％），手术治疗１０１例（５６．１％），死亡８例（４．４％）。Ｂ超对肾损伤病理分型较可靠简便，无损伤，不受病人条件限制，因此建议肾损伤病人首选Ｂ超检查。伴有全腹膜炎的肾损伤不都是腹腔脏器伤所致，约１／３为单纯肾损伤引起。开放性肾损伤病情复杂，多合并肾蒂伤，应积极手术探查。
180 cases of renal injury were reviewed, There were 168 cases of close renal injury, 12 cases of open ones and 70 cases of complex injury including 48 cases with concomitant injury of abdominal viscera(26.7%). 101 patients underwent open operation and 79 treated conservatively. 8 patients(4.4%) died of shock and coexistant injury of other organs. In renal injury with signs of general peritonitis, about 1/3 was solely caused by the renal injury itself with no abdominal visceral injury. Renal pedicle injury was frequently seen in open injury which demands early operative intervention. The accurate evaluation rate of renal injury by Bultrasonography has been 87.8% and reached 100% in severe renal injuries especially when complicated by abdominal visceral injury. Bultrasonography was reliable, simple and noninvasive and was highly recommended for the evaluation of renal injuries.
