高氧半密闭式吸痰法在机械通气患者中的应用 被引量:3

The using of half closed endotracheal suction performed with hyperoxygenation for patients with mechanically ventilation

摘要 目的探讨采用高氧半密闭式吸痰法在机械通气患者中的应用效果。方法将60例机械通气患者按住院时间顺序分成对照组(30例)和观察组(30例),对照组病人采用单纯开放式吸痰方法(注:对照组为方法未改进前的资料)。观察组采用高氧半密闭式吸痰法。高氧即吸痰前先给予100%氧供1min并贯穿整个吸痰过程至吸痰后1min。半密闭式即一次性吸痰管头端与T型套口分别接气管插管(或气切导管)及呼吸机管路,类似三通式吸痰。结果将观察组与对照组2组病人吸痰前后HR、SpO2变化的比较。经t检验,观察组差异无统计学意义(P〉0.05),对照组差异均具有统计学意义(P〈O.05)。同时将两组患者在呼吸机相关性肺炎(VAP)发生率方面比较。经t检验,差异具有统计学意义(P〈0.05)。结论在机械通气患者中采用高氧半密闭式吸痰法能有效地预防,避免因吸痰引起患者HR、SpO2的变化。可以降低VAP发生。 Objective To assess the impact of half closed endotracheal suction performed with hyperoxygenatio for patients with mechanically ventilation. Methods There are 60 patients selected in the hospital according to hospital addmission number, they were graded into comparison group and study group randomly. The comparison group accept the open endotracheal suction performed without hyperoxygenation. The study group accept the half dosed endotraeheal suction performed with hyperoxygenation. Results We compare the result of patients in the two groups, finding that the change in HR, SPO2 after endotracheal suction performed. The difference has statistics sense (P 〈 0. 05) in the comparison group. The difference has no statistics sense (P 〉 0. 05 ) in the study group. At the same time, we compare the patients in the two groups, we find the ventilator associated pneumonia of the comparison group is more than the study group, the difference has statistics sense ( P 〈 0. 05 ) . Conclusion For patients with mechanically ventilation, it has a good sense to take half closed endotracheal suction performed with hyperoxygenation. This measure can reduce the the change in HR, SPO2 after endotracheal suction performed, also reduce the ventilator associated pneumonia.
