摘要目的:观察双水平无创正压通气对慢性阻塞性肺病(COPD)并发呼吸衰竭患者的治疗作用。方法:选择34例COPD伴Ⅱ型呼吸衰竭患者在常规治疗的同时,用双水平正压呼吸机(BiPAP)进行无创通气治疗,观察治疗前、后的临床及动脉血气变化。结果:28例患者经机械通气治疗后意识恢复正常,血氧饱和度(SaO2)逐渐升高,二氧化碳分压(PaCO2)明显下降(P<0.05);2例放弃治疗,4例死亡。结论:BiPAP对COPD并呼吸衰竭疗效肯定,无严重不良反应。 Objective： To observe the effect of noninvasive positive pressure ventilation in the treatment of chronic obstructive lung disease （COPD） with respiratory failure. Methods： 34 COPD patients with respiratory failure were treated by BiPAP noninvasive positive pressure ventilation as taking routine treatment. Clinical symptoms and arterial blood gas were observed before and after treatment. Results： 28 patients got back to normal con- sciousness, with gradually increasing SaO2 and significantly declining PaCO2 （P 〈 0.05 ）. 2 patients abandoned the treatment, and 4 patients died. Conclusion： BiPAP has confirmed therapeutic effect on chronic obstructive lung disease （COPD） with respiratory failure, and has no adverse effect.
