摘要
目的探讨雾化吸入血必净对脂多糖(LPS)诱导Wistar大鼠急性肺损伤(ALI)的治疗作用和临床意义。方法给大鼠尾静脉注射LPS(5mg/kg)复制ALI模型。随机将54只Wistar大鼠分成3组,每组18只。A组(正常对照组):雾化吸入生理盐水;B组(ALI模型组):尾静脉注射LPS即刻开始雾化吸入生理盐水;C组(血必净治疗组):尾静脉注射LPS即刻开始雾化吸入血必净。每组大鼠分别于2h、2d和7d3个时间点各处死6只,按时相观察和收集标本,检测肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、血栓素B2(TXB2)、髓过氧化物酶(MPO)等细胞因子、炎性介质的表达,进行动脉血气分析,测定肺湿重/肺干重、肺含水率,光镜观测肺组织病理变化。结果B组大鼠尾静脉注射LPS后血清TNF-α、IL-6、TXB2及肺组织MPO水平、肺湿/干重量比(W/D)显著高于A组(P<0.05或P<0.01)。而雾化吸入血必净可显著缓解上述变化(P<0.05)。B组PaO2较A组显著降低(P<0.01),PaCO2显著升高(P<0.05);而C组PaO2较B组显著升高,PaCO2显著降低(P均<0.05)。大体标本及光镜观察显示:B组大鼠肺组织病理形态学改变明显,而C组则明显减轻。结论雾化吸入血必净对内毒素所致ALI的大鼠肺组织有明显的保护作用。
Objective To study the therapeutic effect of inhaling Xuebijing aerosol on acute lung injury (ALl) induced with lipopolysaccharide (LPS).Methods The Aid model was built in 54 Wistar rats,which induced by injecting LPS (5 mg/kg) through tail vein. All rats were randomized into 3 groups, including normal control group(A group), of which inhaling saline;ALI model group (B group), of which injecting LPS through tail vein and inhaling saline; treatment group (C group), of which injecting LPS through tail vein and inhaling Xuebijing aerosol. At 2 h, 2nd day and 7th day after model built, 6 rats of each group were put to death and lung section was collected. Detecting the levels of tumor necrosis factor-α(TNF-α, interleukin-6 (IL-6), thromboxane B2 (TXB2) of serum, and expression of myeloperoxidase (MPO) in lung tissue,to measure the arterial blood gas analysis,lung index, wet/dry weight, water ratio, and the pathologic changes in lung were observered with HE stained sections. Results Compared with the normal control group, LPS induced group (B group) exhibited higher levels of TNF-a, IL-6,TXB2 of serum and MPO of rat hmg tissue ( P 〈 0.01 or P 〈 0.05) .Furthermore, the brag index wet/dry weight and water ratio were significanlly higher in B group than A group ( P 〈 0.05). In C group, all indexes obvious improved than other groups ( P 〈 0.05). Additional, the PaO2 decreased in B group ( P 〈 0.01), while PaCO2 increased than A group ( P 〈 O. 05). In C group, the PaO2 increased, while PaCO2 decreased than B group (P 〈 0.05). Observing with general sample and microscope, the pathological change was obvious in B group, which was more severe than C group. Conclusion It demonstrated that inhaling Xuebijing aerosol had protective effect on lung tissue of Aid which induced by endotoxin.
