摘要
目的观察左甲状腺素钠治疗老年原发性甲状腺功能减退症的疗效及不良反应。方法86例原发性甲状腺功能减退老年患者,随机分为治疗组:43例,采用左甲状腺素钠50～100μg口服,1次/d;对照组:43例,采用干甲状腺片治疗80～100mg口服,2～3次/d,两组均治疗6个月。分别于治疗前后观察血清游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、总胆固醇(CHO)、三酰甘油(TG)变化,对比两组临床疗效,观察治疗不良反应。另选30例健康志愿者做为正常对照组。结果治疗前,治疗组及对照组的FT3、FT4均显著低于正常对照组,而TSH、CHO、TG均高于对照组(P<0.01)。治疗后,治疗组及对照组的FT3、FT4均显著高于治疗前,且TSH、CHO、TG均明显下降(P<0.01)。治疗后,治疗组的FT3、FT4明显高于对照组,且TSH、CHO、TG均明显低于对照组(P<0.05)。治疗组总有效率为95.3%,对照组的总有效率为79.1%,两者比较差异有统计学意义(P<0.05)。不良反应:治疗组心动过速2例、胃肠不适1例;对照组组:心绞痛过速5例、胃肠不适1例、头痛1例、过敏反应1例。结论左甲状腺素钠治疗老年甲状腺功能减退症可改善脂代谢,临床疗效优于服用甲状腺素片,无明显不良反应。
Objective To investigate therapeutic effect of elderly patients suffered hypothyroidism in treatment with thyroxine sodium. Methods Eighty-six patients suffered hypothyroidism were randomized into 2 groups： test group（ n = 43） was administered thyroxino sodium, 50- 100μg,po,qd;control group was administered desiccated thyroid tablet, 80- 100mg, po, hid or tid. FT3,FT4, TSH,CHO and TG were measured before treatment and after six-month-treatment later in 2 group which were compared with normal control group.The clinic therapeutic effect was compared between 2 group and side effects were recorded.Results Before treatment, FT3, FT4 of test group and control group were lower than those of nonnal control group and TSH, CHO and TG were higher than those of normal control greup（P 〈 0.01） .After treatment, FT3,FT4 of test group and control group were increased significamtly and TSH, CHO and TG were decreased significantly compared with those of bofore treatment（P 〈 0.01）; FT3, FT4 of test group were increased significantly and TSH, CHO and TG were decreased significantly compared with those of control group（ P 〈 0.05）. The effect rate of test group（ 95.3 % ） was higher than that of control group（ 79.1% ） （ P 〈 0.05 ）. Side effects： tachycardia（ 2 ）, gastro - intestinal（ 1 ）, in test group,and tachycardia（5）,gastro- intestinal（1） ,headache（1）,allergic response（1）in control group. Conclusion Thyroxine sodium is superior to improve hypothyroidism and lipid metabolism in eldedy patients and has little side effects.
