MSCT肺动脉造影联合间接法下肢静脉造影技术探讨 被引量:5

The Study of clinical application of CT pulmonary angiography combining indirect method of lower extremity venography

摘要 目的探讨MSCT肺动脉造影联合间接法下肢静脉造影的技术。方法本研究收集40例临床怀疑肺动脉栓塞患者,按间接法下肢静脉CTV延时时间的不同,随机分为两组,每组各20例。用对比剂跟踪技术(SureStar),先进行MSCT肺动脉造影,然后行下肢静脉造影扫描,A组延迟180~210s扫描;B组延迟240~270s扫描。扫描后分别测量髂总静脉、股静脉、腘静脉的强化CT值。结果40例患者肺动脉均显示清晰。A组髂总静脉CT值平均为115.45±12.95Hu,股静脉CT值平均为110.05±10.82Hu,腘静脉CT值平均为121.85±17.37Hu;B组髂总静脉CT值平均为103.05±13.50Hu,股静脉CT值平均为103.05±8.83Hu,腘静脉CT值平均为111.30±11.79Hu。A组与B组比较存在显著性差异(P<0.05),A组静脉内的造影剂浓度高于B组。诊断肺栓塞18例,合并下肢静脉血栓10例。结论MSCT肺动脉造影联合间接法下肢静脉造影时,应用对比剂跟踪触发扫描技术(SureStar)肺动脉显示良好,延迟180~210s扫描能获得更好的下肢静脉成像效果。 Objective To investigate the clinical application of CT pulmonary angiography combined indirect method of lower extremity venous imaging techniques. Methods Forty patients suspected of pulmonary embolism were randomly divided into 2 groups (group A and B) by different delay CTV time of indirect method of lower extremity, All patients undergone CT Arteriography by constrast medium SureStar technique, After this, ha group A (n = 20), CT venography lower extremity were performed by 180-210s delay time: In group B (n = 20), CT venography of lower extremity were performed by 240-270s delay time. The enhancement CT value of common iliac vein, femoral vein and popliteal vein were measured respectively. Results Pulmonary artery of 40 cases were demonstrated clearly, In group A, the CT mean value of common iliac vein, femoral vein and popliteal vein are 115.45 ± 12.95 Hu, 110.05 ± 10.82 and 121.85 ± 17.37Hu; In group B, the CT mean value of common lilac vein, femoral vein and popliteal vein are 103.05 ± 13.50Hu, 103.05 ± 8.83Hu and 111.30 ± 11.79Hu, The CT value of common ifiac vein, femoral vein and popliteal vein between group A and group B was found statistically significantly different (P 〈0.05). It suggested that contrast concentration of vein in group A was higher than these in group B. 18 cases pulmonary embolism were diagnosed, 10 cases with pulmonary embolism accompanying venous thrombosis were diagnosed. Conclusion Using constrast medium SureStar technique can detect the pulmonary artery clearly, delay 180-210S scan can obtain better image the lower extremity vein.
