关节镜下4股腘绳肌重建前交叉韧带24例临床分析 被引量:1

摘要 目的探讨关节镜下4股腘绳肌重建前交叉韧带的手术方法及疗效。方法2005年12月至2008年6月,收治经临床及关节镜检查确诊的前交叉韧带损伤患者24例,男17例,女7例,年龄19~46岁,平均31.6岁,左膝14例,右膝10例;急性损伤9例,陈旧损伤15例,均为ACL完全断裂。取自体4股腘绳肌,关节镜下行等长重建,股骨端微型钢板固定,胫骨端可吸收界面螺钉固定。参照Lysholm评分及敖英芳临床判断标准评价疗效。结果24例患者切口均一期愈合,无感染及神经血管损伤发生,随访时间6~36个月,平均18个月。术后关节稳定,采用Lysholm膝关节评分方法评估,术前评分为(52.65±9.12),术后评分为(87.73±8.23),与术前比较差异有统计学意义(P<0.01)。参照敖英芳临床判断标准:本组获优16例,良6例,中2例,差0例,优良率为91.67%。结论关节镜下4股腘绳肌重建前交叉韧带方法可行,疗效确切。术后实施正确合理的功能训练也是保证疗效的重要环节。
