摘要
目的探讨硝普钠(sodium nitroprusside,SNP)加入灌注液对移植小肠黏膜结构和功能的早期影响。方法以单袖套法构建SD大鼠节段小肠移植模型为研究对象,将实验动物分成SNP灌注组(实验组)与普通灌注组(对照组),动态检测实验组与对照组植入肠黏膜Na+-K+-ATP酶变化情况、层粘连蛋白在cDNA、mRNA及蛋白等不同水平的表达情况,观察1周内植入肠存活情况与分泌肠液能力,初步分析SNP灌注对植入小肠黏膜的早期影响。结果实验组较对照组造口坏死及不良发生率低(P<0.05)、移植肠及受体生存率高(P<0.05)、黏膜组织Na+-K+-ATP酶含量高(P<0.05),层粘连蛋白在各个水平的表达均有提高(P<0.05)。结论利用SNP进行供肠灌注,能够保护大鼠植入肠黏膜屏障的结构和功能完整。
Objective To evaluate the protective mechanism of sodium nitroprusside (SNP) on mucosal barrier of transplanted intestine. Methods Seventy-two rats were randomly divided into two groups and to establish small bowel transplantation (SBT) models. Rats in control group (n = 14) were furnished with SBT only but not SNP treatment. In SNP group ( n = 22), rats were flushed with SNP during harvesting of graft and SBT were then performed. Changes of Na^+ -K^+ -ATPase in intestinal mucosal tissue and expression of laminin (LN) in three levels such as cDNA, mRNA and protein were detected. The survival rate of stoma in one week and the intestinal secretion were observed to determine the effect of SNP on transplanted intestine. Results There were lower stoma necrosis and unhealthy rate ( P 〈 0.05 ), higher graft and recipient survival rate ( P 〈 0. 05 ), greater quantity of Na^+-K ^+-ATPase in graft rnucosal tissue ( P 〈 0. 05 ) and higher expression of LN in the mucosal barrier of the graft in SNP group than those in control group. Conclusion SNP used as a flushing solution element has reliable effect on the structural and functional integrity of mucosal barrier of the transplanted intestine.
