摘要
目的探讨dGEMRIC用于诊断膝关节软骨早期退变的可行性和价值。方法OA组30例退变膝关节患者经肘静脉注射马根维显后步行10min使对比剂尽量弥散至关节软骨内,注射后2h接受dGEMRIC扫描;对照组27个正常膝关节。于T1时间图中选取感兴趣区(ROI)测量T1值,对照组同时测量软骨浅深层的T1值。对照组与OA组、对照组浅层与深层的T1值行独立样本t检验。结果OA组的T1值为(359.10±41.64)ms,对照组为(426.42±39.81)ms,差异有统计学意义(t=-6.223,P<0.05);对照组髌骨软骨浅层T1值为(411.39±41.50)ms,深层为(468.40±45.05)ms,差异有统计学意义(t=-4.838,P<0.05)。另于OA组5例患者的软骨T1时间图中发现低信号区。结论dGEMRIC通过定量分析关节软骨中GAG含量的变化来判断关节软骨是否有组织成分的丢失,是可用于诊断关节软骨早期退变的有潜力的方法。
Objective To discuss the feasibility and the value of dGEMRIC in diagnosis of early stage of cartilage degeneration of knee joint. Methods Magnevist was injected to 30 patients with osteoarthritis (OA group), the patients were asked to walk for 10 min to make the medium disperse into the cartilage. The patients underwent dGEMRIC scan 2 h later. Twenty-seven healthy adults were selected as control group. T1 value of the ROI was measured in T1 time maps of OA group and control group, so as value of superficial and deeper cartilage of the control group. T1 value of OA group and control group, the superficial and deeper cartilage of the control group were compared with independent-samples t test. Results T1 value of OA group was (359.10±41.64)ms, of control group was (426.42±39.81)ms (t= 6. 223, P〈0.05). T1 value of superficial cartilage was (411.39±41.50)ms, deeper cartilage was (468.40±45.05)ms (t=-4. 838, P〈0.05). Low signal intensity was found in T1 maps of 5OA patients. Conclusion dGEMRIC can be used to diagnose whether the biochemical ingredients of cartilage were lost by means of quantitating the GAG. It has a potential feasibility to diagnose the early stage of cartilage degeneration.
