摘要
目的了解医疗保健机构医务人员对新生儿窒息复苏技术的掌握程度及影响因素。方法在"新生儿窒息复苏培训项目(NRP)"的20个项目省中,每省随机抽取4所不同级别的医院进行调查,共调查80所医院;每所医院随机抽取产科医生、儿科医生、助产士各1名进行操作考核,共调查240名医务人员。结果医务人员操作考核总合格率为72.7%,其中助产士的合格率最高。医务人员在羊水污染时进行气管插管、初步复苏、正压人工呼吸、胸外按压等环节的得分率较低。科室、职称、是否参与新生儿复苏、复苏培训操作练习时间长短以及操作练习教具使用人数的多少等是影响医务人员操作考核得分的主要因素。结论重视复苏理念的建立、强化实际操作训练是新生儿窒息复苏培训的关键。应根据产儿科各层次人员的需求和岗位特点,增加培训内容的针对性。
Objective To study factors that influence the competence of neonatal resuscitation skills among health providers in medical care centers in China.Methods Four hospitals with differing levels of services in 20 provinces of China neonatal resuscitation program(NRP) were selected to participate in the evaluation.Evaluators observed Megacode performance of 240 randomly selected health providers,including obstetric,pediatric,and midwifery staff.Results The overall passing rate of Megacode performance was 72.7%,with midwives demonstrating the highest degree of competence of skills. Providers attained relatively low score in items of performing tracheal intubation if amniotic fluid stained with meconium, initial steps, positive-pressure ventilation, and chest compressions. The difference of hospital section, technical post, whether participate in neonatal resuscitation, time of practice during trainings and number of person using one set of training equipment at the same time when practice are the major factors influencing the Megacode performance score. Conclusion Helping health providers to establish the neonatal resuscitation concept and increase their operation opportunities are the key factors for training of neonatal resuscitation in future. Training contents should be adapted to meet the needs and characteristics of jobs among health providers.
