摘要
目的:探讨胸腔置入中心静脉导管引流联合糖皮质激素治疗结核性胸膜炎的临床疗效。方法:将47例渗出性结核性胸膜炎患者随机分为两组:治疗组24例,采用中心静脉导管置入胸腔引流胸液并口服强的松;对照组23例,采用常规胸腔穿刺抽胸液。两组均采用2HRSZ(E)/(4～6)HR抗结核治疗方案。结果:治疗组胸水消失时间明显短于对照组,发生胸膜粘连、增厚率小于对照组,差异有统计学意义(P〈0.05),两组副反应无明显差异(P>0.05)。结论:胸腔置管引流联合糖皮质激素治疗早期结核性胸膜炎疗效确切,副作用小,值得进一步推广。
Objective To observe the effect of central venous catbeterition combined with glucocorticoid treat tuberculous pleurisy. Methods 47 Tuberculous pleurisy patients were randomly divided into two groups,the treatment group received the treatment of depositing a central venous catheter in pleural cavity and taking prednisone.group B received the treatment of removing fluid from the cavity with pleural centesis intermittently.both groups were gived regular anti-tuberculosis therapy of 2HRSZ （E） /（4-6） HR. Result The incidenc of plura incrassation and pleural adhesions is less in the treatment group, and the time of pleural effusion absorption was shorter in treatment group （P〈0.05） .the incidenc of ill effect was no significant diffrence between the two groups （P〉0.05） . Conclusion It is effective method to treat tuberculous pleurisy by central venous catheter combine glucocorticoid, and ill effect little ,worth to further clinical investigation.
