2μm激光汽化切割术治疗良性前列腺增生的临床研究 被引量:7

摘要 目的回顾性总结使用2μm激光汽化术治疗良性前列腺增生的疗效和安全性。方法共有166例前列腺增生患者接受2μm激光治疗,回顾性总结患者年龄、术前估测前列腺体积、合并疾病情况、手术前后最大尿流率(Qmax)、国际前列腺症状评分(IPSS)变化,手术时间和使用激光能量、主要并发症和术后随访等情况。结果患者平均年龄69.5岁,合并1种或1种以上其他严重疾病者占65%,术前估测前列腺平均体积61.0mL,所有患者均可耐受手术,手术时间平均58min,122例术后2d拔管,25例3d拔管,19例超过3d拔管,平均住院时间8.5d。术后3~9个月随访137例,IPSS评分由(25.2±1.6)分下降到平均(6.3±1.1)分,最大尿流率由(5.2±2.0)mL/s改善为(17.3±3.7)mL/s,手术前后比较差异有统计学意义(P<0.01)。结论2μm激光具有汽化和切割功能,运用2μm激光治疗前列腺增生,具有出血少、痛苦小、术后恢复快、围手术期安全性高、操作简单、并发症少、费用低等优点,是良性前列腺增生患者,尤其是高龄、高危患者较为理想的微侵袭性手术治疗方法。 Objective To evaluate the efficacy and safety of 2μm continuous wave laser vaporesection of the prostate for the treatment of benign prostatic hyperplasia(BPH). Methods A total of 166 patients with benign prostatic hyperplasia received 2μm continuous wave laser vaporesection of the prostate. We investigated the following variables:years, volume of prostate, complication with BPH,max flow,IPSS, operative time, use laser energy, main complication with operation, and follow-up visit time. Results Mean age:69.5 ± 1.5 years. One or more complications occurred in 108 cases(65%). The mean prostate volume was 61.0mL, All patients endured the operation. Mean operation time was 58min. After operations, urethral catheters were indwelled for 2d in 122 cases,3d in 25 cases,and more than 3d in 19 cases,and after the catheters were pulled out,the patients could generally urinate well. The average stay was 8.5d. During 3--9 month follow-up in 137 cases, the mean IPSS decreased from(25.2±1.6) to(6.3 ±1.1)(P 〈0.01) ;Qmax increased from(5.2±2.0)mL/s to(17.3±3.7)mL/s(P〈0. 01). Conclusion 2t^m continuous wave laser has the vaporization and the cutting function, which can yield little blood loss, less pain, and quick recovery, with safety, simplicity and less adverse effects for the patients with BPH,especially titling for advanced age and high risk patients.
