摘要
背景:复合树脂粘接正畸托槽导致牙面釉质损害影响了治疗效果,防治釉质脱矿成为临床正畸医师关注的焦点。目的:对树脂加强型玻璃离子水门汀与新型自酸蚀粘结剂联合用于托槽粘接的性能进行临床评价。设计、时间及地点:临床病例同体对比观察,于2007-04/2008-11在广西中医学院第一附属医院仁爱分院口腔科完成。对象:选择广西中医学院第一附属医院仁爱分院口腔科2007-04/11接受固定矫治器治疗的错牙合初诊患者共36例,660颗牙作为观察对象。方法:每例患者以咬牙合平面与人体中线划分的4个象限中交叉使用不同的粘结剂,即一侧用树脂加强型玻璃离子水门汀联合自酸蚀光固化粘结剂粘接托槽(联合粘接组),另一侧用非调拌型正畸釉质粘结剂粘接托槽(对照组),下颌牙则与上颌相反。主要观察指标:嘱1个月后复诊,其间如有托槽脱落或不适及时复诊,连续观察12个月,每个牙位只统计第一次托槽脱落情况,观察托槽的脱落率和牙面粘结剂残留量。结果:树脂加强型玻璃离子水门汀联合新型自酸蚀粘结剂的脱落率为7.88%,非调拌粘结剂的脱落率为5.45%,两种材料的托槽脱落率差异无显著性意义(P>0.05)。牙面残留粘结剂指数计分显示联合粘接组小于对照组,差异有显著性意义(P<0.01)。结论:树脂加强型玻璃离子水门汀联合新型自酸蚀粘结剂粘接强度可以满足临床要求,对牙齿的保护优于传统复合树脂。
BACKGROUND: The composite resin orthodontic brackets bonding leads to enamel damage to tooth surface and affect the treatment effect, so, it is has been an intense area of focus for investigating how to prevent tooth enamel demineralization during orthodontics. OBJECTIVE: To evaluate the adhesion property of resin reinforced glass ionomer combined with new self-etching adhesive agent. DESIGN, TIME AND SETTING: A randomized comparison observation was completed at the Department of Stomatology, Ren'ai Branch of First Affiliated Hospital, Guangxi College of Traditional Chinese Medicine between April 2007 and November 2008. MATERIALS: Totally 36 cases of newly diagnosed malformation patients used with fixed appliance at the Department of Stomatology, Ren'ai Branch of First Affiliated Hospital, Guangxi College of Traditional Chinese Medicine from April to November 2007 were selected, and 660 teeth were served as the observation object. METHODS: The occlusal surface and the body midline of patients were divided into four quadrant cross using a different adhesive agent, that is, the side of resin reinforced glass ionomer united self-etching adhesive light-curing adhesive brackets (united bonding group), the other side with the use of non-transfer type of orthodontic enamel binder (control group), the mandibular and maxillary teeth are opposite. MAIN OUTCOME MEASURES: The patients were required to further consultation after 1 month. However, during this time, which detached or discomfort bracket should timely referral. The observation continued for 12 months. Only the statistics of each teeth first bracket falling off was analyzed, and the rate of the bracket falling off and adhesive residue of tooth surface was observed. RESULTS: The failure rate of resin reinforced glass ionomer united new self-etching adhesive was 7.88%, and non-transferred with the adhesive was 5.45%, which had no statistical significance (P 〉 0.05). The adhesive remnant index (ARI) points showed it was smaller in the united bonding group compared to the control group, and the difference had significance (P 〈 0.01). CONCLUSION: The bond strength of resin reinforced glass ionomer combined with a newly self-etching adhesive agent can meet the clinical requirements, which has superior protection effect to the traditional dental composite resin.
