摘要
目的利用64层螺旋CT血管成像(CTangiography,CTA)研究甲状腺癌供血动脉的影像学特征及其临床价值。方法收集本院101例(正常组65例,甲癌组36例)符合本研究并行64层螺旋CT血管成像检查病例的颈部原始图像,将数据在AW4.2工作站行容积再现(volumerendering,VR)、最大密度投影(maximum intensity projection,MIP)、多平面重组(multiplanar reconstruction,MPR)及高级血管分析(advanced vessel analyze,AVA)等多种后处理并结合原始图像综合分析甲状腺癌供血动脉特征。结果甲状腺癌组36例肿瘤均由甲状腺动脉(共76支腺体支或非腺体支)供血,所有供血动脉的起源、数目、内径等三维形态学特征均得到清晰显示。将供血动脉来源统计为3型:仅甲状腺上动脉供血型18例(50%);仅甲状腺下动脉供血型6例(16.67%);甲状腺上下动脉均供血型12例(33.33%)。结论甲状腺癌均由甲状腺动脉供血,64层螺旋CTA能较好评价其三维影像特征。
Objective To investigate the three-dimensional imaging features of the feeding arteries in patients with thyroid cancer by using 64-slice helical CT angiography. Methods The scanning data of 101 cases (including 65 cases of normal control and 36 cases with pathologically-confirmed thyroid cancer) who agreed with this investigation and received neck enhanced 64-slice CT were processed on AW 4.2 workstation were collected. Spatial anatomical characters of the feeding arteries in patients with thyroid cancer were observed and analyzed using volume rendering (VR), maximum intensity projection (MIP), multiplanar reconstruction (MPR) and advanced vessel analyze (AVA). Results All the feeding arteries of tumor in thyroid cancer group were thyroid arteries (76 rami). Three-dimensional imaging features of these feeding arteries such as origins, numbers and inner diameters were showed exactly. The resource of feeding arteries had 3 patterns: merely superior thyroid arteries feeding 18 cases (50%), merely inferior thyroid arteries feeding 6 cases (16.67%) and both superior and inferior thyroid arteries feeding 12 cases (33.33%). Conclusion All the feeding arteries of thyroid cancer are thyroid arteries, and 64-slice helical CT angiography can clearly visualize the features.
