摘要
发生在喉腔及喉咽部的平滑肌肿瘤罕见。作者报道了1例喉咽部平滑肌肉瘤和3例喉腔平滑肌肿瘤:1例血管平滑肌瘤、1例平滑肌肉瘤、1例癌平滑肌肉瘤。患者早期症状不明显且无特异性,其临床表现与喉癌极为相似,术前明确诊断有一定困难。头颈部平滑肌肉瘤局部复发率较高,治疗应首选大范围手术切除,因早期颈淋巴结转移极少,对亚临床的淋巴结病变可不予以颈淋巴结清扫术,放疗、化疗可作为姑息治疗手段。
Smooth muscle tumors arise from larynx and hypopharynx is very rare.One case of hypopharyngeal leiomyosarcoma(LMS)and three cases of laryngeal smooth muscle tumors are reported: one a benign angioleiomyoma;one a leiomyosarcoma,another a carcinoleiomyosarcoma.These tumors usually present asymptoms and no characteristics in early stage and are similar to laryngeal squamous cell carcinoma.It is difficult to make a correct diagnosis befort operation.LMS in head and neck showed a high rate of local recurrence and the treatment of choice consisted of primary radical resection including a large part of normal tissue.Early lymph node metastasis is rare.Radical neck dissection is not indicated for subclinic cervical nodal disease.Post-operative radiotherapy and chemotherapy should be considered as a palliative measure.
