血压正常高值人群的胰岛素抵抗和胰岛β细胞功能的变化 被引量:2

Changes of Insulin Resistance and Islet β Cells Function in Subjects with High-normal Blood Pressure

摘要 目的探讨糖耐量正常的血压正常高值人群中血压与胰岛素抵抗和胰岛β细胞功能变化的关系。方法将受试者按照血压水平分为正常血压组和血压正常高值组两组,进行葡萄糖耐量试验,并测量身高、体重、腰围、臀围以及生化指标,计算胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性指数(ISI-composite)和1相Stumvoll指数。结果血压正常高值组腰围、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、HOMA-IR显著高于正常血压组,而1相Stumvoll指数和ISI-com-posite均显著低于正常血压组(P<0.05)。相关分析表明,收缩压与HOMA-IR(r=0.122)呈正相关,与1相Stumvoll指数(r=-0.159)和ISI-composite(r=-0.131)呈负相关(P<0.05)。多元逐步回归分析提示收缩压和甘油三酯是1相Stumvoll指数独立的影响因子。结论在血压处于正常高值的非肥胖人群中,可能存在胰岛素抵抗和胰岛β细胞功能下降。 Objective To investigate the changes of insulin resistance and islet β cells function in subjects with euglycemia and high-normal blood pressure. Methods Total 423 subjects were divided into normal blood pressure group and high-normal blood pressure group. Body height,weight,waist and hip circumference,and biochemical data were measured. Homeostasis model assessment of insulin resistance (HOMA-IR),insulin sensitivity index (ISI)-composite,and first-phase (1PH) Stumvoll index were calculated. Results Waist circumference,total cholesterol,triglyceride,low-density lipoprotein cholesterol,HOMA-IR were significantly higher and 1PH Stumvoll index and ISI-composite were significantly lower in high-normal blood pressure group than in normal blood pressure group(P〈0.05). Systolic blood pressure (SBP) was positively correlated with HOMA-IR (r=0.122) and negatively correlated with 1PH Stumvoll index (r=-0.159) and ISI-composite (r=-0.131) (P〈0.05). SBP and triglyceride were independent factors for 1PH Stumvoll index. Conclusion Insulin resistance and islet dysfunction may exist in subjects with high-normal blood pressure.
