脑出血患者急性期外周血纤溶状态及凝血功能的变化 被引量:8

Changes of fibrinolytic status and coagulation function of peripheral blood at the acute stage in patients with intracerebral hemorrhage

摘要 目的探讨脑出血(ICH)患者急性期外周血纤溶状态及凝血功能的变化。方法检测65例ICH患者(ICH组)发病<24h、第3d及第7d时外周血小板(PLT)数量、体积(MPV)、血浆纤维蛋白原(Fib)及D-二聚体(D-D)水平,并与53名正常对照者(NC组)比较。结果与NC组比较,ICH组发病<24h及第3d时PLT数量明显减少,血浆D-D水平显著升高(均P<0.01),第7d时均恢复至正常水平;发病后各时间点MPV明显增大(均P<0.01);发病第3d、第7d时血浆Fib水平显著升高(均P<0.01)。结论ICH患者急性期存在纤溶活性增高及凝血功能下降的倾向,动态观察上述指标的变化,可指导临床药物治疗。 Objective To explore the changes of fibrinolytic status and coagulation function of peripheral blood at the acute stage in patients with intracerebrul hemorrhage (ICH). Methods The platelet (PLT) count and mean platelet volume (MPV) , the levels of plasma fibrinogen(Fib) and D-dimer(D-D) were detected at 〈 24 h, the 3rd d and the 7th d after onset in 65 ICH patients ( ICH group). The results were compared with 53 normal controls ( NC group). Results Compared with NC group, PLT of ICH group at the 〈 24 h and the 3rd d after onset were significantly lower and the levels of plasma D-D were significantly higher ( all P 〈 0. 01 ) ; PLT and plasma D-D level at the 7th d after onset were recovered to the normal; the MPV in each time points after onset were significant increased ( all P 〈0. 01 ) ; the levels of plasma Fib at the 3rd d and 7th d after onset were significantly increased ( all P 〈 0. 01 ). Conclusions The fibrinolytic activity is increased and coagulation function is decreased at the acute stage in ICH patients. Those indexes observated dynamically may direct the clinical drug treatment.
