绝经后2型糖尿病患者的骨密度变化及其相关性研究 被引量:11

The Change of Bone Mineral Density in Postmenopausal Women with Type 2 Diabetes Mellitus and its Correlative Study

摘要 目的探究女性2型糖尿病患者绝经后的骨密度变化及其与糖尿病的相关性。方法对76例绝经后女性2型糖尿病患者及80例健康的绝经女性用双能X射线吸收法行L2-4、Ward’s三角区、股骨颈、大转子骨密度测量,并测定糖尿病患者空腹血糖、空腹胰岛素、空腹C肽、糖基化血红蛋白等指标。结果①绝经后女性2型糖尿病患者L4、Ward’s三角区骨密度均较健康者显著降低(P<0.05)。②多元回归分析显示:女性糖尿病患者骨密度改变与糖尿病病程、糖基化血红蛋白及绝经年数是负相关(P<0.05),与体重指数呈正相关(P<0.05),与空腹血糖、胰岛素及C-肽等未见相关性。结论2型糖尿病女性患者绝经后骨密度降低较健康女性更为明显。糖尿病的病程和长期的高血糖对骨密度降低有一定作用。 Objective To study the changes of bone mineral density (BMD) in postmenopausal women with type 2 diabetes mellitus and the possible factors involved in. Methods 76 cases of female patients with type 2 diabetes and 80 healthy women received determination of bone mineral density of lumbar spines( L2 -4 ), femoral neck, Ward' s triangle, and trochanter by Dual energy X-ray absorptiometry. Fasting blood glucose ( FPG), fasting insulin, fasting C-peptide, HbAI c of diabetic patient were test- ed. Results Compared with normal group, the BMD of L4 and Ward' s triangle significantly decreased. Multiple linear regression analysis showed that the changes of BMD were negatively correlated with duration of diabetes, HbAlc and postmenopausal years, and positively correlated with BMI. Conclusion The reduction of BMD in postmenopausal women with type 2 diabetes was more significant. Lower BMD was affected by duration of diabetes and long-term poor glucose control.
