Minimally Invasive Percutaneous Internal Fixation with Hollow Screw in the Treatment of Talus Fracture

摘要 目的探讨采用经皮微创空心螺钉内固定治疗距骨骨折的可行性和疗效。方法在C型臂X线机监控下对收治的37例距骨骨折行闭合复位、斯氏针撬拨复位,经皮微创1~2枚4.5 mm空心螺钉内固定,术后辅以支具固定4~6周。结果本组患者有28例获得随访12~24个月(其余失随访)。根据Maryland评分系统进行功能评价,优23例、良4例、可1例。所有患者均无切口感染及皮肤坏死等并发症发生,优良率达到96.4%。结论经皮微创空心螺钉内固定是一种治疗距骨骨折的有效方法,创伤微小,操作简单,方法可靠,能良好地实现骨折复位并且固定牢靠,又可以避免切开复位内固定对骨折周围血供的进一步破坏,可降低距骨缺血性坏死及防止严重后遗症的发生,是一种临床较实用的治疗方法。 Objective To summarize the clinical application and effects of minimally invasive percutaneous internal fixation with hollow screw in the treatment of talus fracture. Methods Thirty seven patients with talus fractures were treated by minimally invasive percutaneous internal fixation with hollow screw. Close reduction was performed under X-ray with C- arm. Percutaneous 1 or 2 hollow screws (4-5 mm wide) were used for internal fixation. Results All patients were followed up for 12-24 months. According to the Malyrand scoring, the results were excellent in 23 cases, good in 4 cases, fain in 1 case. There was no incision infection and skin necrosis. The choiceness rate was 96.4%. Conclusion Percutaneous internal fixation with hollow screw in the treatment of talus fracture has following advantages: the better stability, minimal trauma, simple operation. This method was not necessary to open reduction so that protect the blood supply around the fracture and prevent ischemic necrosis of the talus. It is an effective treatment of talus fracture.
