摘要
目的探讨SDF-1/CXCR4在中青年乳腺癌中的表达与ER、PR、Her-2和BRCA1的表达以及肿瘤的组织病理学、淋巴结转移、5年存活率之间的关系,进一步探讨中青年乳腺癌的预后因素。方法采用原位分子杂交技术在乳腺浸润性导管癌中检测SDF-1基因;采用免疫组化方法,检测乳腺浸润性导管癌中CXCR4、ER、PR、Her-2和BRCA1的表达。结果在49例癌组织中,SDF-1、CXCR4阳性者分别为41和42例,阳性率为83.63%和85.61%,与PR的表达无明显相关性,与ER表达呈正相关,与Her-2表达呈正相关,与肿瘤的组织学分级有明显的相关性,即组织学分级越高,其表达越强。与淋巴结转移也呈明显的正相关,即淋巴结转移的病例阳性表达明显高于未转移病例。与病人的5年存活率呈负相关。结论SDF-1/CXCR4和Her-2高表达是中青年乳腺癌预后不良的重要指标。同时,SDF-1/CXCR4作为促进乳腺癌转移的关键性作用因子,成为了一个乳腺癌治疗的新的靶点。SDF-1/CXCR4与ER及Her-2的同时表达,预后更差。
Objective To investigate the expression of SDF-1/CXCR4 in middle-aged and young patients with infiltrating breast cancer and its correlation with the expression of ER,PR, Her-2 and BRCA1, pathologies of invading breast cancer,metastasis of lymphonodes and 5-years surviving rate. Methods The SDF-1 genes in 49 cases of breast carcinoma tissues aged 26 to 45 years were detected by hybridization in situ. CXCR4,ER,PR,Her-2 and BRCA1 were detected by immunohistochemistry. Results 41 out of 49 cases were positives in SDF-1 and 42 of 49 cases were positives in CXCR4 with the positive rates of 83.63% and 85.61%,respectively. The expression of SDF-1 and CXCR4 was positively with ER,Her- 2,metastasis of lymphonodes and tumors classification and negtively correlated with 5-years surviving rates. Conclusion SDF-1/CXCR4 and Her-2 expressing high positive is important indicating badly factors of breast cancer in middle-young age groups. SDF-1/CXCR4 as a striking point in promoting metastasis of breast cancers,was a new target of treating breast cancer. The prognosis is worse in case of simultaneous expression of SDF-1/CXCR4,ER and Her-2.
