应用岛状皮瓣或游离皮瓣修复严重放射性溃疡 被引量:4

Reconstruction of severe radiation-induced ulceration with island flap or free flap

摘要 目的探讨严重放射性溃疡的修复方法及临床效果。方法自1995年5月至2005年12月,中山大学附属第一医院应用岛状皮瓣或游离皮瓣移植修复严重放射性溃疡25例,其中背阔肌皮瓣3例,侧腹壁皮瓣2例,脐旁皮瓣4例,股前外侧皮瓣6例,胸大肌皮瓣7例,臀大肌皮瓣3例。结果1例皮瓣远端部分坏死,经换药植皮闭合创面,余24例皮瓣全部成活,伤口一期愈合。其中18例随访1~5年皮瓣外观满意,质地柔软,局部未见溃疡复发。结论采用皮瓣或肌皮瓣可以提供较多的组织量,修复溃疡后外观和功能均得到较大改善,是修复严重放射性溃疡可选择的方法之一。 Objective To study the method to repair severe radiation-induced ulceration and evaluate its clinical effects. Methods From May 1995 to December 2005, 25 cases of severe radiation-induced ulceration were treated by flaps or musculocutaneous flaps transfer, in which 3 cases were treated latissimus dorsi myocutaneous flaps, 2 cases by lateral abdominal flap , 6 cases by anterolateral femoral flaps, 7 cases by pectoralis major flap, 3 cases by maximus muscle flap. Results Twenty-four flaps survived with primary healing, 1 flap with partial distal necrosis. The colour and texture of the flaps were excellent, and the outline and functional results were satisfactory after 1 to 5 years follow up. There was no ulcer recurrence. Conclusions These series flaps and musculocutaneous flaps are characterized by large capacity of tissue, good appearance and function. The method is one of optional approach to repair severe radiationinduced ulceration.
