摘要
目的探讨对初产妇实施抚触护理的适宜时间,以提高分娩期的护理质量。方法将180例初产妇随机分为抚触Ⅰ组、Ⅱ组及对照组各60例。对照组实施常规产科护理;抚触Ⅰ组、Ⅱ组在此基础上,分别于子宫颈口扩张1cm、4cm时给予抚触护理。结果三组产时疼痛程度、第一产程时间、总产程时间、产后2h出血量及顺产率比较,差异有统计学意义(P<0.05,P<0.01);抚触Ⅰ组、Ⅱ组与对照组比较,差异有统计学意义(P<0.05,P<0.01),抚触Ⅰ组、Ⅱ组比较,差异无统计学意义(均P>0.05)。结论宫颈口扩张4cm对初产妇实施抚触干预与宫颈口扩张1cm干预效果相同,建议抚触干预从宫颈口扩张4cm开始实施。
Objective To find out the time to start touching intervention for the primipara and to improve the quality of nursing service during labor process. Methods Totally 180 primipara were randomly and equally divided into group Ⅰ , group Ⅱand the control group. The control group were given routine nursing care throughout all stages of labor, while group Ⅰ and group Ⅱ were subjected touching intervention when the cervix dilated 1 cm and 4 cm respectively. Results The severity of pain during labor, the length of the first stage, the length of all 3 stages, the volume of hemorrhage 2 h after labor and the rates of smooth vaginal birth in the 3 groups had significant differences （P〈0.05,P〈0.01）. Significant differences in the above mentioned parameters were found between group Ⅰ and the control group, and between group Ⅱ and the control group（P〈0.05,P〈0.01）. There were no significant differences between group Ⅰ and group Ⅱ （ P〉0.05 for all ）. Conclusion Whenever touching intervention is given to primipara at cervical dilation of 1 cm or 4 cm, the result is similar. In order to maximize the effect of the intervention within minimal work time, touching intervention is recommended to primipara when their cervix dilates 4 cm.
