摘要
目的探讨闭合复位经皮椎体成形术治疗骨质疏松性椎体压缩骨折的临床效果。方法采用闭合复位经皮椎体成形术治疗新鲜骨质疏松性椎体压缩骨折51例,术前VAS疼痛评分(7.60±1.12)分,椎体前壁高度(61.6±22.4)%,椎体后凸角(24.1±10.8)°。先采用闭合复位,然后在C形臂监视下行经皮椎体成形术。术后观察症状改善情况、椎体高度恢复率、后凸矫正率、骨水泥渗漏情况。结果51例手术顺利完成。49例术后72h内腰背疼痛缓解或显著缓解,2例无效,总有效率96%,VAS评分(1.8±0.91)分(P<0.05)。椎体前壁高度平均恢复20.9%(P<0.05)。椎体后凸角平均矫正12.6°(P<0.05)。椎旁骨水泥渗漏7例。结论闭合复位经皮椎体成形术治疗骨质疏松性椎体压缩骨折,不仅能迅速缓解疼痛,也可恢复椎体高度,改善后凸畸形。
Objective To study the clinical effect of closed reduction and percutaneous vertebroplasty in the treatment of osteoporotic vertebral compression fracture.Methods Closed reduction and percutaneous vertebroplasty was used to treat 51 cases of fresh osteoporotic vertebral compression fracture.The average VAS score was(7.6±1.12) points,average height of vertebral body was(61.6±22.4)%,average Cobb's angle was(24.1±10.8)degree.All cases were carried out closed reduction first,and then vertebroplasty guided by C-shape X-ray machine.The symptom improvement,recovery rate of height of vertebral body,restoration rate of Cobb's angle and bone cement leak were observed after operation.Results 51 cases were finished successfully.The total effective rate was 96%,though 2 cases were invalid,backache improved or obviously improved in other 49 cases within 72 hours after operation,average VAS score was(1.8±9.1)points(P〈0.05).Restoration rate of average height of vertebral body was 20.9%(P〈0.05),Cobb's angle improvement was 12.6 degree(P〈0.05),bone cement leak in 7 cases.Conclusion Osteoporotic vertebral compression fracture could acquire rapidly pain release,restore height of vertebral body and improve abnormality by means of closed reduction and vertebroplasty.
