运动训练对急性心肌梗死后心室重构的影响及氧化应激的作用 被引量:9

Effect of exercise training on ventricular remodeling in rats with acute myocardial infarction

摘要 目的:观察急性心肌梗死(AMI)后运动训练对心室重构及左室泵功能的影响,探讨氧化应激对细胞外间质的作用及可能通路。方法:SD大鼠随机分为假手术组(Sham组,n=10)、心梗对照组(AMI-Sed组,n=10)和心梗+运动训练组(AMI-Ex组,n=10)。结扎冠脉建立AMI模型,AMI-Ex组进行8周跑台训练。术后9周测量血流动力学参数及胶原容积分数,测量心肌超氧阴离子、过氧化氢、血管紧张素Ⅱ(AngⅡ)含量,还原型辅酶Ⅱ(NADPH)氧化酶活性、基质金属蛋白酶-2(MMP-2)蛋白量及细胞核因子-κB(NF-κB)蛋白量。结果:与AMI-Sed组比较,AMI-Ex组胶原容积分数、右心室心脏重量指数、肺含水率和左心室压力负最大值明显降低(P<0.05),左心室压峰值及左心室压力变化速度正负最大值显著升高(P<0.05)。心肌超氧阴离子、过氧化氢含量、AngⅡ含量、NADPH活性及MMP-2与NF-κB蛋白量显著降低(P<0.05)。结论:AMI后运动训练可以抑制左心室重构,改善左室泵功能。其机制可能与抑制AngⅡ-NADPH氧化酶-ROS-NF-κB-MMP-2通路相关。 Objective To investigated the effect of exercise training, started early after acute myocardial infarction (AMI), on LV remodeling and dysfunction in the mouse, and to study the role of oxidative stress and extracellular matrix. Methods The AMI rat model was achieved by ligating coronary artery. The animals were randomized into Sham group (n = 10), AMI-Sed group (n = 10), and AMI-Ex group (n = 10). After 9 weeks of treatment, hemodynamic changes, morphological parameters and collagen volume fraction (CVF) were assessed. The contents of superoxide anion, hydrogen peroxide, angiotensin II , NADPH oxidase activity, nuclear factor kappaB (NF-KB) and matrix metalloproteinases 2 expression were detected. Results As compared with AMI-Sed, CVF, pulmonary congestion and LVEDP were reduced in AMI-Ex group (P 〈 0.05), and LVSP and + dp/dt max were improved (P 〈 0.05). We also found that lower oxidative stress in trained mice, and this may associated with reduce in Ang II level and NADPH oxidase activity (P 〈 0.05). Furthermore, we found that NF-KB protein level was decreased in AMI-Ex, which was accompanied by decreased in MMP-2 (P 〈 0.05 ). Conclusions Exercise training in mice after AMI has benefit effect on LV remodeling and pumps function. Exercise decrease Ang II , which may trigger MMP2-mediated ECM degradation. These effects may proceed through inhibition of NADPH oxidase-indueed oxidative stress and the subsequent NF-KB activation.
