MSCT多时相薄层扫描诊断微小肝癌的临床应用研究 被引量:8

摘要 目的探讨微小肝癌(MHCC)的检查方法及诊断价值。方法27例微小肝癌患者分别行多层螺旋CT(MSCT)多时相薄层扫描,扫描时间分别为动脉期20-25s、门脉期50-55s、平衡期2-3min、延迟期5-7min,多时相薄层3-5mm,用高压注射器经肘静脉注射,速率为3ml/s,剂量为1.5-2.0ml/kg。结果共发现36个病灶,动脉期、门脉期、平衡期和延迟期的检出率分别为91.7%、72.2%、77.8%、83.3%,综合多时相薄层扫描检出率为94.4%;DSA检出MHCC的敏感性为33.3%。结论多层螺旋CT(MSCT)多时相薄层扫描,检出MHCC以动脉期和延迟期为最佳,综合多时相薄层扫描可以提高MHCC的检出率和诊断准确性。 Objective To study the methods and diagnostic value of microhepatic cellular carcinoma(MHCC) with multi-slice helical computed tomography(MSCT).Methods 27 patients with microhepatic cellular carcinoma were diagnosed by clinical,and underwent multi-phase helical CT with 3-5mm thick-section scan.For multi-phase helical CT,hepatic arterial-phase(HAP) and portal venous-phase(PVP) and equilibrium-phase(EP) and delayed-phase(DP) scanning began 20-25s,55-55s,2-3min,5-7min.respectively,after intravenous nonionic contrast material injection at a rate of 3ml/s.Results 36 lesions were found.The sensitivity was 91.7% at HAP,72.2% at PVP,77.8% at EP,83.3% at DP,94.4%at triple-phase and 33.3%in DSA.Conclusion MHCC were well detected with the HAP and DP images by Multi-phase helical computed tomography scanning.However,all three phase scan are recommended for detection of MHCC.
