摘要
目的 评价直丝弓矫治器排齐整平阶段Nance弓加强支抗的三维表现.方法 选择中度以上拥挤,上颌减数双侧第一前磨牙的患者41例,随机分组,20例不用辅助支抗装置作为对照组,21例粘Nance弓作为Nance组.排齐整平前后取模型并拍头颅侧位片.模型用Roland LPX-1200三维点激光扫描仪数字化,使用测量软件(Rapidform 2006(R))处理分析治疗前后上颌第一磨牙各牙尖标志点三维向上的线距及磨牙角度变化.并对头颅侧位片的前牙变化进行测量.应用SPSS10.0统计软件对测量结果行统计学分析.结果 两组患者排齐整平阶段前牙唇倾控制没有显著统计学差异.在矢状向、横向及扭转的控制上Nance组与对照组有显著的统计学差异.以中央窝点为例,对照组磨牙有1.269 mm的近中移动,Nance组有0.537 mm的近中移动(P<0.01);对照组磨牙有0.522 mm的腭向缩窄,Nance组有0.145 mm扩宽(P<0.001);对照组磨牙出现3.172°近中颊向扭转,Nance组有0.402°扭转(P<0.01).结论 Nance弓在控制磨牙的矢状向位置、宽度和扭转上是可靠的.
Objective To evaluate the anchorage loss in first premolar extraction cases with Nance appliance three-dimensionally during the leveling and alignment stage. Methods Forty-one orthodontic patients were selected. Enrollment criteria included moderate to severe crowding and extraction of upper first premolars. The patients were randomly divided into two groups. Twenty cases without anchorage control served as control group and 21 cases with Nance appliance served as experimental group. Lateral cephalograpms and study casts were collected before treatment and at the end of leveling and alignment stage. The dental casts were scanned with LPX-1200 three-dimensional laser scanner and analyzed using three-dimensional reverse modeling software program (Rapidform 2006(R)). Statistical analysis was performed using software package Spss 10.0. Results No significant difference was found in upper incisor inclination between control group and the experimental group. Significant difference was found in sagittal displacement, transverse movement and the rotation control of upper first molars between experimental and control groups. Conehtsions Nance appliance was effective in controlling sagittal displacement, transverse movement and rotation of the upper first molars.
