动态监测cystatin C对肾移植排斥反应的诊断价值

摘要 目的研究动态监测血清半胱氨酸蛋白酶抑制剂C(cystatin C,Cys C)对肾移植排斥反应的诊断价值。方法36例肾移植患者,稳定组29例,排斥组7例,20例健康体检者作为对照组。对术后稳定组和排斥组的Cys C和Cr、Ure、UA进行术前、术后不同时间点的动态监测并测定术后肾小球滤过率(GFR)。结果移植术前,稳定组和排斥组的各项指标均显著高于对照组(P<0.01);稳定组Cys C、Cr、Ure从术后1 d起均显著低于术前水平(P<0.01)。Cys C下降最明显(42.9%),UA则从术后3 d起显著低于术前水平(P<0.01)。与稳定组比较,术后排斥组的Cys C、Cr、Ure、UA分别升高了120.0%、91.8%、93.8%和86.9%,且Cys C的变化最早发生;Cys C、Cr、Ure、UA与GFR均呈显著负相关(P<0.01),Cys C与GFR的相关系数(r=-0.935)最大;ROC曲线下面积(AUCROC)Cys C(0.933)明显优于其他指标(P<0.01)。结论Cys C能较其他指标更早期、灵敏地判断肾移植排斥反应。
