摘要
目的:探讨经皮内镜下胃造瘘术(PEG)在重型颅脑损伤患者的临床应用价值。方法:对我科收治的19例PEG术的重型颅脑损伤患者临床资料进行回顾性分析。结果:治愈7例,好转9例,植物生存1例,死亡2例。19例患者均造口成功,引流通畅,成功率100%,并顺利实施肠内营养,营养状态较前明显好转。2例术后出现低热,造瘘口周围皮肤局部红肿,经换药及抗生素治疗2天体温恢复正常。1例使用胃造瘘注入流质饮食时造成导管堵塞,经用糜蛋白酶8000U+温生理盐水50ml反复冲洗后导管得以疏通。7例置管1.5~4.5个月,患者神志恢复并可自主进食后拔管,平均拔管时间3.2个月。结论:PEG行肠内营养在临床应用中具较多优点,是重症颅脑外伤患者肠内营养的一种重要途径。
Objective:To explore the effects of perculaneous endoscopic gastrostomy(PEG) in the patient with severe cerebral trauma.Methods:Retrospective analysis was performed on 19 cases of craniocerebral trauma.Results:Seven patients were cured,nine patients became better,one became plants survival,2 cases died.The PEG operation was success in 19 patients and there were no severity complications.The patients with slight complications,including infection of incisional wound,ductus obstruction,were also rapidly cured.7 cases brought the catheter about 1.5~4.5 months.We took the catheter when the patients recovered consciousness and could feed themselves.Average decannulation time was 3.2 months.Conclusion:Percutaneous endoscopic gastrostomy has more advantages in enteral nutrition in clinical application.This is a kind of important ways of enteral nutrition in the patients with severe craniocerebral trauma.
