摘要
目的探索未来战争中闭合性肝脏破裂的早期诊断方法 ,为及时分级救治、挽救生命、减少并发症、降低病死率提供依据。方法回顾分析笔者所在医院136例闭合性肝破裂。其中术前明确诊断为肝脏破裂109例中CT检查97例,超声检查116例,分别确诊94、93例;其余均为手术探查确诊。未确诊者,及时行腹腔穿刺抽出不凝性血液后术中探查确诊。结果痊愈出院135例,1例死于多脏器功能障碍综合征。17例出现并发症者均经治愈出院。结论外伤性肝破裂的临床表现易被合并其它伤的症状掩盖,早期诊断较困难;CT、超声检查及腹腔穿刺有助于早期诊断。重视肝破裂的早期确诊,及时合理治疗有利于减少并发症和降低病死率。
Objective To research for the early diagnosis of the closed hepatorrhexis in the future fight,in order to provide closed hepatorrhexis evidence to rescue by stages,save lives,decrease complications,lower case fatality ratio.Methods Retrospective analysis was made in the 136 cases of the closed hepatorrhexs who were operated on from 10.1999 to 07.2009 in anthor＇s hospital.Results Admitting diagnosis of closed hepatorrhexs was in 109 cases（70.58%）,CT examination in 97 cases;final comfirmed diagnosis in 94（96.91%） cases;ultrasonograph,116 cases,final confirmed diagnosis in 93（80.17%） cases,the rests were on operation for final confirmed diagnosis.Conclusion The clinical manifestation of the liver traumatic rupture is complicated with other wounds,it is hard to do the early diagnosis.
