踝部骨折不同内固定方式的生物力学研究与临床应用 被引量:48

摘要 为了临床选择有效的内固定方式提供实验依据。用21具新鲜尸体踝部标本,制成三种骨折模型,用4~5种常用内固定方式置于WD-10E万能电子力学试验机上,测定每种内固定抗剪切、抗扭转强度并进行比较。钢板及加压螺丝钉固定强度大,普通螺丝钉及克氏针固定强度小。临床手术171例中129例平均随访4.2年。优良率分别为:加压螺针为90.2%、钢板87.5%、张力带87.5%,普通螺钉76.2%,克氏针77.1%。认为加压螺钉内固定具有创伤小、固定确切的特点,是一种安全可靠的方法。 The study was conducted to provide experimental relevant data of clinical internal fixation method for choice. Twenty-one fresh cadvearic ankle joints were imitated. Internal fixation methods in four or five were tested on WD-10E machines.Both anti-shear and anti-torsion strength were measured and compared.The fixation instensity of cortical screw and Kirscher' s-wires was the smallest, and that of plate and cancellous screw was the greatest. In 171 postoperative ankle joint fracture cases, 129 cases were followed up with average time of 4.2 years, in which the good rates of cancellous screw, plate, tension band wire fixation were 90.2% , 87.5%, 87.5%, respectively. Cancellous screw fixation is a simple and effective internal fixation method for ankle fractures.
