摘要
目的评价封闭负压吸引（vacuum—sealingdrainage，VSD）治疗四肢大面积皮肤损伤的效果。方法对收治的外伤致四肢大面积皮肤缺损患者67例，自2003年3月至2007年3月的患者32例分为传统治疗组，2007年3月至2009年3月收治的35例为VSD治疗组。VSD治疗组急诊行“清创术加VSD覆盖术”，术后行抗破伤风、抗感染对症支持治疗，肉芽组织生长满意后植皮。传统治疗组行常规清创、包扎、换药、抗感染、抗破伤风治疗，待肉芽组织生长满意后植皮。比较VSD治疗组和传统治疗组住院时间、创面微生物生长情况、植皮所需时间、植皮存活率，分析差异有无统计学意义。结果VSD能有效保护创面，减少细菌感染，缩短植皮所需的时间，减少住院时间，植皮存活率高。结论VSD是治疗四肢皮肤缺损的有效措施。
Objective To evaluate the effect of vacuum - sealing drainage （VSD） in treating large area traumatic cutaneous defects. Methods Sixty - seven patients with large area limbs traumatic cutaneous defects were collected from March 2003 to March 2009, including 32 patients during March 2003 and March 2007 as traditional treatment group, and 35 patients as VSD treatment group. VSD treatment was adopted in VSD group besides debrigement, support treatments such as antitetanus and anti - infection treatments were given, and when granulation tissue grew well, skin - grafting was done. De- brigement,wrapping,and changing dressing were carried out in tranditional treatment group, and skin - grafting was done only if the granulation tissue grew well. The hospital days, growth of microorganisms on the raw surface, the time needed for skin -grafting, survival rate of skin -grafting were analyzed. Results VSD could protect the raw surface effectively, decrease bacterial infection, shorten the grafting time and the hospital days, and survival rate of skin - grafting was high. Conclusion VSD is an effective method to cure large area limbs traumatic cutaneous defects.
