冠心病合并2型糖尿病患者冠状动脉病变特点及与高敏C-反应蛋白关系分析 被引量:5

摘要 目的探讨冠状动脉粥样硬化性心脏病(冠心病)合并糖尿病患者冠脉病变特点及与高敏C反应蛋白(hs-CRP)的关系。方法选择冠状动脉造影确诊为冠心病的患者219例,根据是否合并糖尿病分为糖尿病组(T2DM组)(n=67)和非糖尿病组(无T2DM组)(n=152),根据冠脉病变支数分为单支病变、双支病变及三支病变并测定hs-CRP值。结果 T2DM组的hs-CRP水平明显高于无T2DM组(P<0.05),糖尿病组双支及三支冠脉病变的构成比明显高于无T2DM组(P<0.01),但不同冠脉病变支数间的hs-CRP水平无差异(P>0.05)。结论冠心病合并糖尿病患者体内炎性反应水平较非糖尿病者高,患多支病变的几率明显增大,但冠脉病变支数与hs-CRP关系不密切,仅靠hs-CRP水平不能预测冠心病的冠脉病变支数。 Objective To investigate the characteristics of coronary arterial lesion in patients with coronary heart disease (CHD) complicated with type 2 diabetes mellitus (T2DM) and its correlation with high-sensitive C reactive protein(hs-CRP). Methods 219 patients with CHD identified by coronary angiography were divided into two groups according to whether they were complicated with T2DM,group 1 (DM group)was complicated with T2DM and group 2( NDM group) was without T2DM. They were divided into one-vessel lesion, two-vessel lesion, three-vessel lesion group according to the counts of coronary arterial lesion vessels, and the levels of hs-CRP were detected. Results The levels of hs-CRP in DM group were significantly higher than those in NDM group( P 〈0.05), the proportion of two-vessel lesion and three-vessel lesion in DM group was more than that in NDM group( P 〈 0.01 ). There was no significant difference in hs-CRP levels among lesion vessels ( P 〉 0.05 ). Conclusion The inflammatory reaction in patients complicated with T2DM is obviously higher than that without T2DM,and the ratio of multi-vessel lesion is increased. The correlation between the counts of coronary arterial lesion vessels and the hs-CRP levels is not close, and the detection of levels of hs-CRP can' t predict the counts of coronary arterial lesion vessels.
