摘要
目的探讨评价血清糖类抗原(carbohydrate antigen,CA)125诊断上皮性卵巢癌(epithelial ovarian cancer,EOC)的意义。方法收集并分析2006年1月1日至2008年12月30日中山大学附属第三医院妇科收治的104例卵巢癌标本。对卵巢癌标本按照国际妇产科联盟(International Federation of Gynecology and Obstetrics,IFGO)对上皮性卵巢癌进行分期。统计学分析血清CA125对卵巢上皮性癌的诊断价值。结果血清CA125>35U/mL诊断上皮性卵巢癌的敏感性和阳性预测值分别是86.2%和83.3%。当血清CA125取值升高时,特异性和阳性预测值增高。上皮性卵巢癌ⅢC期中,细胞低分化组的血清CA125值高于细胞高分化组,差异有显著意义(P<0.05)。血清CA125>150U/mL时,提示晚期卵巢恶性肿瘤的敏感性、特异性和诊断准确性都较好,分别为85.4%、77.3%和78.8%。结论血清CA125在上皮性卵巢癌的组织学分类、细胞分化及和临床分期诊断中具有一定的意义。
Objective To evaluate the significance of serum CA125 in diagnosis of epithelial ovarian carcinoma.Methods The clinical data of 104 cases of epithelial ovarian carcinoma were collected and analyzed in the Third Affiliated Hospital of SUN Yat-sen University from January 2006 to December 2008.Stages of specimens were classified according to the standard of International Federation of Gynecology and Obstetrics.Diagnosis value of serum CA125 in ovarian cancer diagnosis were analyzed by statistical analysis.The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Third Affiliated Hospital,SUN Yat-sen University.Informed consent was obtained from all participates.Results When serum CA125 were〉35 U/mL,the sensitivity and positive predictive value were 86.2% and 83.3%,respectively for diagnosis of epithelial ovarian carcinoma,and the specificity and positive predictive value were higher when serum CA125 cut-off were higher.In the stage ⅢC group,serum CA125 were higher significantly in low-differentiation group than that in the higher-differentiation group (P〈0.05).When serum CA125 was〉150 U/mL,the sensitivity,specificity and accuracy were better for the diagnosis of advanced malignant ovarian tumor,which were 85.4%,77.3% and 78.8%,respectively.Conclusion Serious CA125 was useful in diagnosis of epithelial ovarian carcinoma including histology,differentiation and stage.
