摘要
目的探讨阅读障碍(RD)儿童发育异常的生物测量指标及其评定方法。方法根据遗传发育神经生物学理论和有关文献编制发育异常评定量表(DAS),对101例汉族RD儿童和66名正常儿童作发育异常的评定分析。结果RD儿童各项评分均高于正常儿童(P<0.01)。部分手项目评分在RD儿童中存在性别及左右侧的差异。DAS的重测信度达0.98,评定者一致性Kappa=0.92。评定灵敏度为84.6%~92.9%;特异度86.3%~94.1%。结论阅读障碍儿童较多存在胚胎期神经发育不良。DAS对神经发育异常评定的灵敏度、特异度、一致性和可靠性较好。
Objective To study the developmental anomalies in the children with reading disorder (RD), and to establish a development anomalies scale (DAS) for clinical practice. Methods DAS consisting of 36 physical and dermatoglyphics items was rated in 101 RD children (70 males and 31 females) and 66 controls (40 males and 26 females). Results RD children were significantly higher in all scores of the developmental anomalies items than controls ( P <0.01). There were significant differences between males and females, left and right hands in some hand′s items in RD group, but not in controls. The test retest reliability coefficient was 0.98, Kappa value of the raters was 0.92. The diagnostic sensitivity ranged from 84.6% to 92.9% and the specificity was from 86.3% to 94.1%. Conclusion RD children have more prenatal neurodevelopmental anomalies. And DAS could be used clinically as an effective measure for neurodevelopmental anomalies.
