部分卵子卵胞浆内单精子注射避免体外受精失败的价值探讨

The value of partial oocytes ICSI to avoidn the failure of IVF

摘要 目的探讨部分卵子卵浆内单精子注射(ICSI)的必要性。方法对2007年1月至2008年12月98个治疗周期患者的同胞卵分别实施常规体外受精(IVF)和卵浆内单精子注射,对两组受精率、卵裂率比较,分析。结果 (1)IVF组受精率47.08%,ICSI组受精率86.61%,两组比较,ICSI组高于IVF组,差异有统计学意义(P<0.01)。98周期妊娠率41.0 5%,种植率26.07%;(2)IVF组全部受精失败1 9例,占1 9.39%,低受精(受精率≤25%)7例,占7.1 4%,不受精和低受精总和占26.53%,1CSI组没有不受精患者;(3)IVF不受精组由于移植了ICSI组胚胎妊娠率31.58%,而IVF/ICSI均受精组妊娠率43.42%,IVF组不受精组妊娠率低于IVF/ICSI均受精组,但两组差异无统计学意义(P>0.05);(4)9 8周期IVF组可利用胚胎203枚,胚胎利用率5 7.1 8%,而ICSI纠可利用胚胎5 1 9枚,胚胎利用率75%,两组相比,差异有统训学意义(P<0.01)。结论对于不明原因的不孕人工授精3次以上未孕、疑有IVF受精障碍患者和男性精液临界值的患者应及时行同胞卵IVF和ICSI,避免全卵受精失败和受精率低下。 Objective To analyze the necessity of performing ICSI to the partial eggs retrieved from IVF patients. Methods Total.of 98 IVF patients from Jan. 2007 to Dec.2008 were recruited in the study. Patients with partial eggs were fertilized by conventional IVF and by ICSI respectively. Fertilization and cleavage rates of conventional IVF and ICSI procedure were comparatively compared. Results (1) Fertilization rates were 47.08% and 86.61% in 1VF group and ICSI group respectively (P 〈0.01). Pregnant rate was 41.05% and implantation rate was 26.07%. (2)Total of 19 cases (19.39%) had no fertilization in IVF group, and 7 cases (7.14%) had fertilization rate less than 25%. Patients in ICSI group had no fertilization failure. (3)The pregnant rate of patients in IVF failed group, who transferred with embryos by ICS1, was 3 1.58%., however the pregnant rate of patients with egg fertilization from both in IVF and ICSI was 43.42% (P〉 0.05). (4)Utilized embryos were 203 (57.18%) in IVF group and 519 (75%) in ICSI group, respec ively (P〈0.01). Conclusion For unexplained infertility patients with three times IUI failure,, with suspicious of IVF failure, or poor sperm quality and quantity, partial IVF and partial ICSI should be applied for improving poor fertilization in conventional IVF.
