摘要
目的 探讨米索前列醇片、地西泮联合笑气用于人工流产术中镇痛效果及安全性.方法 选取自愿要求终止妊娠的早孕妇女104例为实验组,术前口服米索前列醇片及地西泮、术中吸入笑气作为人工流产术中镇痛;同时选取自愿要求终止妊娠的早孕妇女102例为对照组,该组术前不做特殊处理.用随机单盲方法 对两组的镇痛效果、宫颈口松弛度、人工流产综合反应、手术时间、术中出血量等进行观察比较.结果 实验组与对照组中扩宫效果的满意、一般、困难之间比较有显著性差异（χ^2别为88.08、8.45、135.05,均P〈0.05）.实验组与对照组在人工流产手术中的镇痛效果（显效、有效、无效）（χ^2别为77.29、51.48、194.19）、人工流产综合反应发生率（χ^211.11）、手术时间（t=-16.29）进行比较均有显著性差异（均P〈0.05）.两组术中出血量比较无显著性差异（χ^20.05,P〉0.05）. 结论 米索前列醇片、地西泮联合笑气用于人工流产镇痛效果可靠,并能预防人工流产综合反应及缩短手术时间,方法 简单、安全,值得普及推广.
Objective To investigate analgesic effects and safety of misoprostol and diazepam combined with nitrous oxide in operation of induced abortion. Methods 206 women who requested voluntarily to terminate pregnancy in early period were divided into study group （n = 104） and control group （n = 102）. 104 women in the study group orally took misoprostol and diazepam before abortion and then inhaled nitrous oxide in operation to relieve pain, while 102 women in the control group didn＇t take any medicines and only inhaled nitrous oxide in operation to relieve pain. Analgesic effect, cervical relaxation, incidence of artificial abortion syndrome, operation time, intraoperative blood loss were observed and compared in random single-blind method. Results There were significant differences in cervixdilating effects （satisfactory, general and difficult）, analgesic effects （effectual, effective and ineffective）, incidence of artificial syndrome and operation time between the study group and the control group （x^2 = 88.08, 8.45, 135.05, 77.29, 51.48, 194.19, 11.11, - 16.29, all P 〈 0.05 ）. No differences were observed in mean intraoperative blood loss （x^2 = 0.05, P 〉 0.05 ）. Conclusion The analgesic effect of misoprostol and diazepam combined with nitrous oxide is reliable for induced abortion, also, it can prevent incidence of artificial abortion syndrome and shorten the operation time. It is simple and safe, so, it can be widely used in clinic.
