摘要
目的对比是否合并冠心病的糖尿病患者的动态心电图特点,明确12导联同步动态心电图糖尿病合并冠心病患者的意义。方法采用分层(分类)抽样法,随机抽取24h12导联同步动态心电图检测的130例糖尿病患者,根据冠状动脉造影结果分为冠心病组(Ⅰ组)72例和非冠心病组(Ⅱ组)58例,对比分析两组发生心律失常情况及心率变异性时域指标,对其中室性早搏患者对比室性早搏发生部位及窦性心率震荡指标。结果两组房性心律失常发生率差异不具有显著性(P>0.05),但室性心律失常发生率差异具有显著性(P<0.05);两组心率变异性时域指标差异具有显著性(P<0.05);Ⅰ组左心室前壁室性早搏发生率(38.9%)高于Ⅱ组(16.7%),窦性心率震荡指标两组对比差异具有显著性(P<0.05)。结论糖尿病合并冠心病患者与非合并冠心病患者相比,室性心律失常发生率明显增高,左心室前壁室性早搏发生率高,心率变异性降低,窦性心率震荡减弱。12导联同步动态心电图包括心率变异性及窦性心率震荡检测对糖尿病患者,特别糖尿病合并冠心病具有重要意义。
Objective To contrast the features of the dynamic electrocardiograms of patients with diabetes mellitus（DM）whether or not complicated with coronary artery disease（CAD）in order to predict the risk of CAD in DM patients.Methods Totally 130 DM patients determined by 24h 12-leads dynamic electrocardiogram （DECG） were divided into two groups：72 cases of CAD （groupI） and 58 cases of patients without CAD （groupII）,contrast the arrhythmia and heart rate variability （HRV） indices in the two groups,contrast the original positions where the ventricular premature beats occurred and heart rate turbulence （HRT） indices in each case with ventricular premature beat.Results The distinction of auricular arrhythmia was not significant in the two groups（P0.05）,but the distinction of ventricular arrhythmia was significant（P0.05）;The distinction of HRV：SDNN、SDANN Index and SDNN Index were significant in the two groups（P0.05）;The rate of ventricular premature beats from left anterior wall in group I （38.9%）was significant higher than that in group II（16.7%）.The distinction of HRT indices TO and TS was significant in the two groups（P0.05）.Conclusion In DM patients with CAD the incidence of ventricular arrhythmia and the rate of ventricular premature beats from left anterior wall was significant higher than that in DM patients without CAD.HRV was significant lower,HRT was weaken.The determination of 12-leads DECG including the analysis of locating the ventricular premature beats and HRV and HRT was special significance in deciding DM patients whether or not complicated with CAD.
