摘要
目的介绍第二足趾游离移植再造拇指建立两套动脉血供的新方法。方法游离的第二足趾移至受区后,在鼻烟窝处切断桡动脉,将足背动脉与桡动脉近端吻合,第一趾足底总动脉外侧支与桡动脉远端吻合。临床应用11例。结果11例再造拇指全部成活,术后均无血循环危象发生。术中发现桡动脉近、远端和游离趾动脉吻合后,能即刻通过掌深弓恢复全手血供。结论该法是游离足趾移植再造拇指时建立两套动脉血供的良好方法,并可以达到不影响手部血供的目的。
Objective To introduce a
new method to build up double blood supply system in second toe to thumb transfer.
MethodsZAfter the second toe with its vascular pedicle was harvested and positioned to the
hand, the radial artery was cut in the snuff box area. The toe was then revascularized by
anastomosing the dorsalis pedis artery to the proximal end of the radial artery, and suturing the
lateral branch of the first common plantar digital artery to the distal end of the radial artery. The
procedure was performed in 11 patients clinically. Results Intraoperatively,restoration of the
blood supply of the hand via the deep palmar arch was promptly observed when the two cut
ends of the radial artery were anastomosed to the vessels of the toe. There was no
postoperative circulation crisis. All the reconstructed thumbs survived. Conclusions This
method of revascularization in toe to thumb transfer was not only a simple and effective
method to achieve double blood supply system for the transplant, but it also had the advantage
of not influencing the blood supply of the hand.
