摘要
目的 探讨异丙酚对学龄前儿童静脉滴注氯胺酮药代动力学的影响。方法 16例3.5~6 岁行择期手术的学龄前儿童随机分为两组:Ⅰ组为氯胺酮组,Ⅱ组为氯胺酮+ 异丙酚组。Ⅰ组采用异氟醚吸入诱导,Ⅱ组采用异丙酚诱导。气管插管后两组均静脉滴注氯胺酮3m g·kg- 1 ·h- 1(0.05% ),同时吸入异氟醚行麻醉维持。应用反向高效液相色谱法测定氯胺酮血药浓度,采用中国药理学会编辑的3P97软件计算药代动力学参数。结果 两组房室模型均拟合为二房室开放模型。药代动力学参数:Ⅰ组T1/2 α= (0.15±0.04)h,T1/2 β= (2.84±0.44)h,Cl= (0.34±0.06)L·kg- 1 ·h- 1,Vd= (0.66±0.29)L·kg- 1 ;Ⅱ组T1/2α= (0.14±0.06)h,T1/2β= (2.67±0.25)h,Cl= (0.33±0.08)L·kg- 1 ·h- 1 ,Vd= (0.67±0.22)L·kg- 1 。各药代动力学参数两组间无显著性差异。结论 异丙酚不影响氯胺酮在学龄前儿童体内的代谢。
Objective To determine the effect of propofol on the pharmacokinetics of ketamine infusion in pre school children.Methods Sixteen patients were randomly assigned to group Ⅰ(Ketamine)and group Ⅱ (Ketamine+Propofol).After intubation,0.05% Ketamine was infused to both groups at the rate of 3 mg·kg -1 ·h -1 ,The plasma concentration of Ketamine was measured using high performance liquid chromatography.Pharmacokinetic parameters were calculated using 3P 97 software composed by Chinese Pharmacologic Sociaty.Results Both groups were of two compartment open model.Group Ⅰ:T 1/2 α=0 15±0 04h,T 1/2 β=2 84±0 44h,Cl=0 34±0 06L·kg -1 ·h -1 and Vd=0 66±0 29L·kg -1 ;Group ⅡT 1/2 α=0 14±0 06h,T 1/2 β=2 67±0 25h,Cl=0 33±0 08L·kg -1 ·h -1 ,Vd=0 67±0 22L·kg -1 There is no significant difference in pharmacokinetic parameters between the two groups.Conclusion Propofol does not have any effect on the pharmacokinetics of ketamine infusion in pre school children.
