Three dimensional sonographic reconstruction of intrahepatic vascular structures

摘要 目的探讨实质性脏器内血管结构的三维超声重建方法及其临床应用前景。方法对10例正常肝脏和9例异常肝脏的肝内血管进行三维超声重建,所用仪器为韩国Medison公司的KretzVoluson530D三维超声成像系统,探头为三维容积探头,频率3.5~5.0MHz。重建模式选用透明成像最小回声模式、表面成像模式及混合模式。结果用透明成像最小回声模式重建的肝内血管三维图像,其血管树连续性好,无中断现象;可显示末端细小的分支或属支;血管形态规整,边缘平滑;血管间空间关系明确;能整体显示肝内大血管,类似其真实解剖关系。透明成像模式与其它成像模式如与X线模式或表面模式组合,能同时识别肝内血管结构与周围病变组织的空间毗邻关系。用表面成像模式重建的血管三维图像,可清晰显示血管的内腔及其内壁结构特征,并可显示血管分叉处的开口。 Objective\ To evaluate the methodology and the clinical value of three dimensional (3D) sonographic reconstruction of the vascular structures in parenchymatous organs. Methods\ 10 normal livers and 9 abnormal livers were studied. The instrument used was Kretz Voluson 530D 3D imaging system of Medison, Korea. The transducer used was 3D volume transducer with frequency of 3.5~5.0 MHz. The rendering mode were transparency minimun, surface imaging and mixed rendering mode. Results When rendered by transparency minimun mode, the vascular trees of 3D images were continuous, the spatial relationship among the vascularities was definite, the fine branches or tributories could be displayed well. When the transparency minimun mode was combined with X ray mode or surface rendering mode, the relationship between the intrahepatic lesions and the intrahepatic vessels could be demonstrated clearly. The structural feature of lumen, internal wall and the opening of the bifurcation of vessels could be demonstrated when the 3D images were reconstructed by the surface rendering mode. Conclusions\ The 3D images of the intrahepatic vessels will be of clinical significance for the diagnosis and the treatment of the intrahepatic vascular diseases.
