摘要
目的:探讨血清癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、鳞状细胞癌抗原(SCC-Ag)和细胞角蛋白19(CYFRA21-1)联合检测在肺癌诊断方面的价值。方法:采用ELISA法检测52例肺癌患者和88例肺良性疾病患者血清CEA、NSE、SCC-Ag和CYFRA21-1水平。结果:肺癌组血清CEA、NSE、SCC-Ag和CYFRA21-1水平(中位数)分别为3.41、7.97、0.86和1.84μg/L,高于肺良性疾病组血清CEA、NSE、SCC-Ag和CYFRA21-1的水平2.58μg/L(WilcoxonW=5452.043,P<0.001)、3.11μg/L(WilcoxonW=4572.538,P=0.006)、0.33μg/L(WilcoxonW=5408.051,P<0.001)和1.02μg/L(WilcoxonW=4642.574,P=0.002)。CEA检测肺腺癌、NSE检测肺小细胞癌、SCC-Ag和CYFRA21-1检测肺鳞状细胞癌的灵敏度分别为60.0%、66.7%、72.0%和80.0%。4项肿瘤标志物联合检测诊断肺癌的灵敏度为97.48%。结论:CEA对腺癌、NSE对小细胞肺癌、SCC-Ag和CYFRA21-1对肺鳞癌具有较大的辅助诊断价值,4项联合检测可提高肺癌诊断的灵敏度。
Aim:To explore the diagnostic value for lung cancer of combined determination of serum carcinoembryonic antigen(CEA),neuron specific enolase(NSE),squamous cell carcinoma(SCC-Ag) and cytokeratin fragment antigen 21-1(CYFRA21-1).Methods:The four tumor markers in serum were detected by ELISA of 52 patients with lung cancer and 88 patients with lung benign disease.Results:The levels of CEA,NSE,SCC-Ag and CYFRA21-1 of lung cancer patients were 3.41,7.97,0.86,and 1.84 μg/L,respectively,which were higher than those of control group 2.58 μg/L (Wilcoxon W=5 452.043,P0.001),3.11 μg/L (Wilcoxon W=4 572.538,P=0.006),0.33 μg/L (Wilcoxon W=5 408.051,P0.001) and 1.02 μg/L (Wilcoxon W=4 642.574,P=0.002).The sensitivity values of CEA for lung adenous cancer,NSE for small cell lung cancer(SCLC),SCC-Ag and CYFRA21-1 for squamous cell carcinoma were 60.0%,66.7%,72.0% and 80.0%.The sensitivity value of the four tumor marker combined to diagnose lung cancer was 97.48%.Conclusion:Determination of CEA in adenocarcinoma,NSE in SCLC,SCC-Ag and CYFRA21-1 in squamous cell carcinoma has great diagnostic value.A combination of the 4 markers may increase diagnostic value in terms of its sensitivity.
