胃癌组织凋亡相关蛋白Bcl-xL和Bax的表达及其临床意义 被引量:4

摘要 胃癌的发生、发展不仅与癌基因和(或)抑癌基因失衡相关,亦与细胞凋亡调控异常相关。目的:探讨凋亡相关蛋白Bcl-xL和Bax在胃癌发生、发展中的作用及其临床意义。方法:收集胃癌手术标本57例、相应癌旁组织57例和距肿瘤组织5 cm以上的正常胃黏膜组织57例,同期收集正常胃窦黏膜组织30例作为正常对照组。采用免疫组化技术检测Bcl-xL和Bax在胃癌组织、癌旁组织、正常胃黏膜组织中的定位和表达情况,分析胃癌组织Bcl-xL和Bax的相关性及其与胃癌主要临床病理特征的相关性。结果:胃癌组织Bcl-xL阳性表达率和Bcl-xL阳性高表达率显著高于正常胃黏膜、慢性非萎缩性胃炎、慢性化生性萎缩性胃炎、异型增生组织;胃癌组织Bax阳性表达率和Bax阳性高表达率显著低于正常胃黏膜、慢性非萎缩性胃炎、慢性化生性萎缩性胃炎、异型增生组织,差异均具有统计学意义。胃癌组织Bcl-xL和Bax表达呈负相关(rs =-0.302,P=0.022);胃癌组织Bcl-xL和Bax的阳性表达率和阳性高表达率与肿瘤分化程度呈正相关(P<0.05),与患者的性别、年龄、浸润深度、淋巴结转移、临床分期均无关(P>0.05)。结论:胃癌组织Bcl-xL表达增强,其表达与Bax呈负相关。胃癌组织Bcl-xL和Bax表达与肿瘤分化程度呈正相关,可作为胃癌患者预后判断的指标之一。 The development and progression of gastric cancer were not only related to the imbalance between oncogenes and tumor suppressor genes, but also the dysregulation of apoptosis. Aims: To investigate the role and clinical significance of apoptotic-related protein Bcl-xL and Bax in the development and progression of gastric cancer. Methods:57 specimens of gastric cancerous tissue, 42 specimens of chronic non-atrophic gastritis, 39 specimens of chronic metaplastic atrophic gastritis, 33 specimens of dysplasia and 30 biopsy specimens of normal gastric mucosa from healthy controls were collected. Immunohistochemical method was used to detect the locations and expressions of Bcl-xL and Bax. The relationship between Bcl-xL and Bax in gastric cancer and their relationship with major clinicopathological features of gastric cancer were analyzed. Results:The expression rate and high expression rate of Bcl-xL in gastric cancer tissue were significantly higher than those in normal gastric mucosa, chronic non-atrophic gastritis, chronic metaplastic atrophic gastritis and dysplasia, while the expression rate and high expression rate of Bax in gastric cancer tissue were significantly lower. The expression of Bcl-xL in gastric cancer was negatively correlated with expression of Bax (rs =-0.302,P=0.022). The expression rate and high expression rates of Bcl-xL and Bax were positively correlated with the differentiation of gastric cancer (P〈0.05), while not correlated with patients’ gender, age, infiltration degree, lymph node metastasis or clinical stage (P〉0.05). Conclusions:The expression of Bcl-xL was increased in gastric cancer, which was negatively correlated with the expression of Bax. The expressions of Bcl-xL and Bax in gastric cancer were positively correlated with the differentiation of cancer, and may be used as an indicator of prognosis for gastric cancer.
