摘要
目的探讨新生儿缺氧缺血性脑病(HIE)的发病原因。方法回顾性分析1998年1月—2008年10月收治的HIE56例临床资料,对其病因、临床表现、辅助检查等相关因素进行分析。结果全部病例均有生后窒息史,宫内窘迫22例(39.29%);母亲患妊娠高血压综合征11例(19.64%);脐带绕颈12例(21.43%),第二产程延长7例(12.50%)。Apgar评分0～3分22例,4～7分34例。窒息持续时间<4min29例,5min～10min23例,>10min4例。HIE轻度39例,中度14例,重度3例。合并低血钙5例,低血糖10例。结论 HIE的根本原因是围生期窒息缺氧,主要诱因是孕母患妊高征及异常生产史。HIE表现分度与5minApgar评分呈正相关,与窒息程度及持续时间关系密切。因此,做好围生期保健,预防围生儿缺氧因素的发生,及时正确的复苏等,可降低HIE发病率、致残率、病死率。
Objective To study the causes of neonatal hypoxic-ischemic encephalopathy.Methods A retrospective analysis from January 1998 to December 2008 admitted to HIE 56 cases.The etiology,clinical manifestations,laboratory examinations and other related factors were analyzed.Results The data showed that all patients had asphyxia after birth.There were 22 cases of fetal distress（39.29%）,mothers suffered from pregnancy-induced hypertension syndrome in 11 cases（19.64%）,Cord around neck in 12 cases（21.43%）,the second stage of labor extending in the 7 cases（12.50%）.Apgar score 0-3 points in 22 cases,4~7 points in 34 cases.Asphyxia duration less than 4min in 29 cases,5~10 min in 23cases and more than 10 min in 4 cases.There were 39 cases of mild HIE,moderate in 14 cases,moderate and severe in 3 cases.There were 5 cases combined hypocalcemia and low blood sugar in 10 cases.Conclusion The cause of HIE is perinatal asphyxia and the main incentive is the pregnant mother suffering from pregnancy-induced hypertension and abnormal production history.HIE performance of sub-degree and 5 min Apgar score was positively correlated with the degree and duration of asphyxia close.Therefore,to do perinatal care,prevention of perinatal hypoxia factors occur,timely and correct recovery and so on,can reduce the incidence of HIE,morbidity and mortality.
