摘要
目的评价载药国产纳米活性炭在胃癌根治术中的淋巴示踪及淋巴化疗作用。方法 92例进展期胃癌患者随机分成载药组、示踪组和对照组。载药组术前12h经胃镜在肿瘤周围注射国产纳米活性炭-5氟尿嘧啶混悬液,示踪组术前12h经胃镜在肿瘤周围注射国产纳米活性炭混悬液,比较根治术中淋巴结清扫的数目、转移度、淋巴结的黑染、转移及细胞凋亡情况,观察不良反应。结果载药组和示踪组每例患者清扫淋巴结数明显高于对照组,淋巴结微转移率明显高于对照组;黑染淋巴结中发生淋巴结转移的阳性率明显高于未黑染的淋巴结及对照组;载药组淋巴结细胞凋亡率明显高于示踪组和对照组。载药组和示踪组局部注射纳米活性炭未见有局部及全身不良反应。结论胃癌病灶周围局部注射国产纳米活性炭-5氟尿嘧啶混悬液对根治术中转移淋巴结的清除,尤其是对微转移淋巴结的清除有重要意义;有淋巴化疗作用。
Objective To estimates the spike-effect and the lymphatic chemotherapy-effect of domestic drug-loaded nanometer activated carbon in the radical operation of the carcinoma of stomach.Methods 92 patients with advanced gastric carcinoma were randomly divided into three groups:load group,spike group and control group.Load group received 5-fluorouracil-loaded Carbon Nanoparticles Suspension Injection in tumor vicinity by the gastroscope 12h before the operation.Spike group received Carbon Nanoparticles Suspension Injection in tumor vicinity by the gastroscope 12h before the operation.To compare the number,the metastasis degree and black-stained of lymph node and cellular apoptosis resettled in the radical operation.To observe adverse drug reactions.Results The average number of the resected lymph node was higher in load group and in spike group than in control group.The micrometastasis rate of lymph node was higher in spike group and in load group than in control group.The metastasis rate of lymph node was higher in black-stained lymph node of spike group and load group than in unstained lymph node of spike group,load group and control group.The lymphocyte apoptosis rate of load group was higher in load group than in spike group and in control group.No local and systemic adverse reactions happened after nanometer activated carbon local injection.Conclusions Local injection of 5-fluorouracil-loaded nanometer activated carbon around the tumor is important to the metastasis lymph node resection,especially to the tiny metastasis lymph resection in the gastric radical operation.It has lymphatic chemotherapy-effect.
