TGF-β1等指标的变化在HIE中的临床意义 被引量:3

摘要 目的 探讨新生儿缺氧缺血性脑病的血清转化生长因子β1与外周血白细胞和血小板计数的动态变化情况,了解这些指标与疾病严重程度的关系,为病情的判断和诊治提供依据.方法 选取2004年3月~2005年1月在西安交通大学医学院第一附属医院新生儿科就诊的缺氧缺血性脑病患儿为实验组,同期出生的足月新生儿为对照组,检测生后第1、3、7天血清中转化生长因子β1的浓度,同时检测外周血白细胞和血小板计数,以及新生儿神经行为评分.结果 生后第3天血清中转化生长因子β1的浓度在组间比较差异有统计学意义(F=3.592,P<0.05),且组间两两比较差异亦有统计学意义;只在新生儿出生后第3天血清转化生长因子β1与血小板计数之间呈正相关(r=0.251,P<0.05),而与白细胞计数之间第1、3、7天均无明显相关性(均P>0.05);缺氧缺血性脑病组生后第1、3、7天血清转化生长因子β1的浓度与第14天新生儿神经行为评分呈正相关(均P<0.05).结论 血清转化生长因子β1在缺氧缺血所致的神经元死亡中起保护性作用;血小板可作为临床判断缺氧缺血性脑病患儿病情严重程度和监测病情变化的参考指标. Objective To investigate dynamic changes in serum level of transforming growth factor-betal ( TGF-β1 ), white blood cell count (WBC) and platelet count (PLT) of neonates with hypoxic-ischemic encephalopathy (HIE) and explore their relationships with severity and prognosis of the disease. Methods 56 full-term newborns with HIE treated in Department of Neonatology, The First Affiliated Hospital of Medical College, Xi' an Jiaotong University over a period from March, 2004 to January, 2005 were included in experimental group and 30 normal full-term newborns born in the same period were included in the control group. The serum level of TGFβ1 of all neonates in the two groups at 1st, 3rd and 7th day after birth was detected dynamically and WBC and PLT in peripheral blood were counted at the same time. Also, all neonates were scored in NBNA. Results In serum level of TGFβ1 at 3rd day, there was statistically significant difference as compared with that at 7th day after birth in each group and between the two groups (F =3. 592,P 〈0.05). Only at 7th day after birth, the serum level of TGFβ1 was positively correlated with count of PLT ( r = 0. 251, P 〈 0.05 ) , but there was no statistically significant correlation between the serum level of TGFβ1 and count of WBC at respective 1 st, 3rd and 7th day after birth (all P 〉 0.05 ). The serum levels of TGFβ1 at 1st, 3rd and 7th day after birth of neonates in the experimental group were positively correlated with their scores in NBNA at the 14th day after birth(all P 〈0.05). Conclusion Serum level of TGFβ1 may play a protective role in HIE. The count of PLT can be used as a reference index to judge severity of the disease and monitor changes in patient' s condition.
